Nobody prepares you for what PCOS asks of you.

The diagnosis is one sentence. Living with it is a hundred quiet adjustments that nobody else sees. The tiredness that sleep will not fix. The appointments. The waiting. The trying again. And explaining it repeatedly to people who are certain it is one simple thing.

And most of you were told to lose weight and come back.

I have heard that in my room more times than I can count, from women who had been saying for years that something was wrong and were sent away with the same sentence every time. Some of them stopped going back at all, which I understand completely, and which is exactly what the system was counting on.

So I am not going to tell you to trust them. I am going to tell you not to disappear. Get your bloods done, keep the appointment even when it feels pointless, not because they have earned it but because PCOS has quiet parts that only ever show up on a test. Stay on their list. And then build the rest of your life without waiting for them.

And it does get better. I have sat with a lot of women who got much more of their life back than they thought they were going to, and not one of them got there by finally trying hard enough. They got there because at some point they stopped doing it on their own.

And then there is the tiredness, which nobody treats, because it is not a symptom on anybody’s list. You just carry it while you manage everything else.

That is the part I can help with. I want to be clear about it, because I have seen accounts in this niche be far less careful with you. Hijama is not for your hormones and I am not going to pretend otherwise. It is somewhere to be looked after, where you do not have to explain yourself first.

Make du’a. Loads of it. Ask Allah ﷻ for shifa, because every means we have only works by His permission.

You deserved better than what most of you got. You are more than a diagnosis 🤍

Every week someone asks me how often they should be having hijama, and almost every week they’re asking the wrong question.

Because there are two questions inside it, and they have completely different answers.

If you’re maintaining, keeping ahead of things, generally well and want to stay that way, then a rhythm is enough. For most people that’s two to four times a year. You’re not fixing anything, and there’s no prize for going more often.

If you’re treating something, that’s a different conversation entirely. Chronic pain, something that’s been building for years, something that isn’t shifting. That needs an actual plan built around you, and at the start it’s often more frequent than a maintenance rhythm. But that gets decided between us, after I’ve asked you a lot of questions, not from a schedule you found online.

A lot of people also like to follow the Sunnah days, which the Prophet ﷺ pointed to, and there’s something lovely about that. It takes the deciding out of it.

The one thing I’ll say for certain: more is not better. Your body needs the time in between. Rest is part of the treatment, not the gap between treatments.

Recovery is not the reward you get for training. It is part of the training.

The stimulus happens in the session. The adaptation happens afterwards, while you are asleep, eating properly, and not doing very much. Skip that half and you have done the difficult part for no return, which is why most people who plateau are not undertraining. They are under-recovered, and their response is to add volume, which is the single intervention guaranteed to make it worse.

The boring things do most of the work here. Sleep, protein, real rest days, deload weeks you actually take. Nothing replaces those, including me.

On top of a routine that already works, there is hijama. The Prophet ﷺ was cupped and said there is healing in it, athletes have used cupping for a very long time, and I am not going to downplay that to sound more like a lab. What I will say is that if your sleep is five hours and your rest days are theoretical, fix that first, because nothing I do will outrun it.

If you do book, timing matters. Not the day before you compete, not straight after a hard session, and keep the following 24 hours light.

We tell girls about the first one at eleven years old and then, seemingly, decide the rest is none of their business.

So it arrives sideways. Sleep that breaks at three in the morning. Joints that ache for no reason. A temper that surprises you. The word that disappears halfway through the sentence. And because nobody named any of it in advance, a lot of women arrive at the conclusion that something is wrong with them personally.

The part I hear most is not the symptoms. It is being disbelieved. By work, sometimes by family, and eventually by themselves.

Please see a doctor, and go in specific. Write down what has changed and when it started. Ask directly about your options, HRT included. If you are not taken seriously, ask to see someone else. That is allowed and it is not rude.

And here is the thing nobody has offered you. Everything on the table is about correcting something. Tests, doses, adjustments, what you should be eating and when. All of it useful, and all of it about fixing you.

Nobody has offered you anything that isn’t about that. A body running on two years of broken sleep is carrying a great deal, and being properly looked after, by someone who isn’t trying to correct anything, is not a small thing.

People ask a lot of questions before a session and almost none afterwards, which is the wrong way round.

The session itself is the easy part. What you do for the day after it is where most of the difference gets made, and the instruction people ignore most is the simplest one. Rest. No intense exercise for a day or two, light movement after a couple of hours is fine, and no, you do not feel well enough for the gym this evening.

Keep the marks dry for a full 24 hours. No shower, no bath, no swimming. Somebody always asks whether a quick one counts. It does not.

Black seed oil on the marks twice a day, water steadily rather than all at once, and stay away from saunas, steam and extreme cold for a day or two. Eat light. Go gently on red meat, dairy, fried food and sugar, and lean on soups, fruit and vegetables and lean protein instead.

The marks fade at different speeds for different people and it means nothing about how well it worked, so please stop comparing yours to photos on the internet.

And if you feel faint or unwell, or an area is getting more sore rather than less, speak to a doctor and message me. Do not sit at home wondering.

Some people arrive with a folder.

Notes on their phone, a timeline, a list of everything they have already tried, all of it prepared in advance in case I ask them to justify themselves. They have been through enough appointments to expect that they will have to.

That is what pain without a diagnosis does. The tests come back clear and instead of relief you get something worse, because “we cannot find anything” gets heard by everyone around you as “there is nothing there.”

And eventually you stop bringing it up. Not because it stopped, but because explaining it every time costs more than just carrying it quietly. That is the part almost nobody says out loud, and it is the loneliest bit of the whole thing.

Please keep going with the doctors. Ask for the second opinion, ask for the referral, take someone with you if being believed is easier with a witness in the room. The people who get answers are usually the ones who kept asking.

And undiagnosed is not the same as imaginary. I believe you. You should not have needed to read that from a stranger.

People ask me which is better, cupping or massage or physio, and I always want to ask them better at what.

They do different jobs. A physio assesses how you move and gives you a plan. A massage therapist works on soft tissue and, honestly, on the state you walk in with. Hijama is a prophetic practice that many people take up as one part of how they care for a body that has been carrying a lot.

None of those sentences is a ranking.

Here is the part that will not surprise anyone who has been with me a while. There are people I send away. Pain that is new, severe or getting worse, anything after an injury, anything with numbness or weakness in it. That is a doctor or a physio, and it is a doctor or a physio first, not after you have tried me.

The best results I see are rarely from one thing on its own. They are from people who worked out what was actually wrong and then chose accordingly.
