Most people don’t realize there’s a third option.

Not the rushed 10-minute appointment. Not the $500/month concierge retainer that usually still requires you to carry insurance. Something different.

Direct Primary Care means a flat monthly membership, same-day appointments, wholesale lab pricing, and your doctor’s actual cell phone number. No surprise bills. No insurance required to walk through the door.

Don’t get me wrong, concierge medicine offers a great experience, but you’re paying for it twice. DPC gives you the relationship without the premium price tag.

Affordable. Accessible. Always here when you need me.

Welcome!

I am Dr. Dakota Boisseau, a board certified family medicine physician and owner of River Direct Primary Care.

I believe the system is broken. Patient should not have to wait hours to only see their doctor for 10 minutes. You should not wait weeks to see your doctor for just a quick question. I believe an open communication, spending time with my patience, and being there when they need me. I opened River direct primary care to help people become healthier, happier, and to make healthcare, transparent and affordable again.

Menopause Monday: Why can’t I sleep?!?

During perimenopause and menopause, declining estrogen disrupts melatonin production. Falling progesterone removes a natural calming effect on the brain. Night sweats fragment your sleep before you even realize they’re happening.

The good news: this is one of the most treatable parts of the menopause transition. Hormone therapy, CBT-I, and a few targeted lifestyle changes can genuinely transform your sleep not by masking the problem, but by addressing the root cause.

If you’ve been white-knuckling your way through exhausted days, you don’t have to keep doing that.
This is exactly what we work through together at River DPC. No rushed appointments, no waiting weeks to be seen, just real answers and a plan that actually fits your life.

FAQ Friday: Does magnesium actually help you sleep?

Short answer — yes. But the form you’re taking matters.
Magnesium bisglycinate has the strongest clinical evidence for sleep, backed by a 2025 randomized controlled trial showing real improvement in insomnia scores. The cheap magnesium oxide you’ll find at most big box stores? About 4% absorption. Basically expensive nothing.

At River DPC, this is exactly the kind of conversation we have time for. No rushed appointments, no 10-minute visit where supplements don’t even come up. Just you and your doctor actually talking through what works for your body.

GLP-1s work. But are you losing fat, or muscle?

The number on the scale is going down. But here’s what most people aren’t asking: what exactly are you losing?

Studies show that up to 15-40% of weight lost on GLP-1 medications like semaglutide and tirzepatide can come from lean muscle mass, not just fat.

Muscle isn’t just for the gym. It drives your metabolism, protects your joints, keeps you strong as you age, and helps your body stay insulin-sensitive. Lose too much of it, and you can end up lighter on the scale but metabolically worse off and at higher risk of regaining the weight as fat if you ever stop the medication.

The good news? This is largely preventable.

The two most evidence-backed strategies:
✨Eat enough protein. Higher protein intake signals your body to preserve muscle during caloric restriction.
✨Resistance train. Lifting weights (or body resistance work) tells your body the muscle is needed and to keep it.

Brain fog during menopause is real and it’s one of the most common symptoms nobody warned you about.

Forgetfulness. Trouble concentrating. Feeling like you’re moving through mud.

More than two-thirds of women experience this during the menopause transition, and most assume the worst.

Here’s what the research actually shows: your core cognitive abilities are intact. The brain fog is real, but it is not permanent damage. It is a hormonal and lifestyle-driven symptom which means there are real things you can do about it.

Start here:
∙ Prioritize sleep above almost everything else. Hormonal sleep disruption makes cognitive symptoms significantly worse.
∙ Move your body daily. Aerobic exercise is one of the most evidence-backed tools for brain health during this transition.
∙ Eat to support your brain. Blood sugar stability, protein, and omega-3s matter more than you think right now.
∙ Reduce your stress load where you can. Psychological stress compounds hormonal cognitive symptoms in a real, measurable way.

And if you’ve brought this up with a doctor and felt brushed off, that’s not okay. You deserve a doctor who has time to actually dig into what’s going on.

That’s what Direct Primary Care is built for.

A question I’ve been hearing more often lately: “Can I just get a blood test for Alzheimer’s now?”

The short answer is yes — and no.

Earlier this year, the FDA cleared the first blood test for Alzheimer’s assessment in primary care settings. It’s now available nationwide for patients 55 and older who are experiencing symptoms of cognitive decline.

For a long time, evaluating cognitive symptoms meant referrals to specialists, PET scans, or spinal taps. A simple blood draw that can be done in your doctor’s office changes the conversation.

But here’s the part that tends to get left out of the headlines: this test is designed to rule out amyloid plaques — a hallmark of Alzheimer’s disease. It is not a standalone diagnostic tool, and a positive result is not a diagnosis.

Blood tests must be interpreted in clinical context and confirmed with additional testing when treatment decisions depend on the result.

And perhaps most importantly: if your memory occasionally slips but you’re still managing your medications, finances, and daily activities independently, routine biomarker testing is not currently recommended and may cause more harm than good.

The right question isn’t “can I get this test.” It’s “should I get this test — and what would we do with the result?”

If you or someone you love has concerns about memory or cognitive change, come talk to me. That’s what I’m here for.

If your GLP-1 has stopped moving the scale, your medication didn’t fail you. Your body adapted. That’s different and it’s fixable.

Here’s what’s happening:
When you lose weight, your metabolism becomes more efficient. Your body burns fewer calories at your new size. Add in subtle changes in eating habits and potential muscle loss from reduced appetite, and the plateau makes complete biological sense.

The good news: clinical trials show GLP-1 medications can reduce body weight by 15–22% on average. But that result requires more than just taking the medication.

Protein intake of at least 1.2–1.6g per kilogram of body weight per day is essential to preserve muscle. Resistance training 2–3x per week protects your metabolic rate. And real-world data shows that more than half of people stop GLP-1 therapy within a year, most often because they didn’t have clinical support to manage what comes after the easy early losses.

That’s what I am here for!
