We participate in Medicare, Medicaid and accept Private Pay Residents. We provide skilled care, intermediate care and have an Alzheimer/Dementia wing. We are family owned and operated facility providing quality care and services for residents by a caring, compassionate and professional staff.

We were honored to be invited to Wallace State College to represent all 5 Rehab Select facilities. We spoke to 160 nursing students who are graduating in August! It was a wonderful blessing to meet all of the bright, intelligent students who are excited to be taking a new step in their lives! We can’t wait to welcome you to one of our AMAZING families! The smiles, laughter, and joy was ringing through our halls from a fun game of cheese head! Our Residents laughed until we cried throwing cheese balls every where. The Staff and family members may have had more fun than expected with this game! This is the blessings we live for! The memories, family having fun, smiles, and pure love is what makes every day a blessing!
