Raw wide-awake Motiva Preservé.

She sits up, looks down, and immediately lights up because for the first time, everything looks even. Natural, balanced, and exactly what we planned.

One quick note: right after surgery, things can look a bit more full because of the numbing solution used during a wide-awake case. Over the next days and weeks, that settles and the breasts soften into their true shape.

Wide awake isn’t for everyone, but for the right patient it’s an incredibly controlled, empowering experience with a smooth recovery.

Preservé is minimally invasive, but it’s not “plug and play.” Experience matters.

There’s a tactile component to the procedure, ultrasound guidance, and a real learning curve just like any surgical technique. The more cases you do, the better your judgment, planning, and consistency become.

After 100+ Preservé cases, my results are extremely consistent, my planning process is methodical, and I recommend it to the vast majority of my first-time breast augmentation patients because I believe it’s a superior option to traditional augmentation for the right candidate.

Wide awake breast augmentation can be an incredible experience for the right patient, but it has to be done the right way.

In my practice, “wide awake” does not mean cutting corners to save money. The anesthesiologist is still present to start the IV, give IV antibiotics, monitor you closely, and if you suddenly feel anxious or decide you don’t want to be awake anymore, we can transition to sedation immediately.

The goal is a calm, controlled experience with the safest setup possible and a clear head afterward.

A lot of people hear “Preservé” and assume it’s just a standard over-the-muscle (subglandular) augmentation. It’s not.

Traditional subglandular augmentation creates a pocket by separating the breast off the underlying fascia. The implant sits beneath the breast tissue, and the native ligament support system is essentially above the implant rather than actively supporting it.

Motiva Preservé is a breast tissue preservation approach. Instead of “peeling” the breast off the chest wall, we develop the pocket within the posterior layer of the breast itself using fluid-based expansion and specialized instrumentation. The goal is to preserve Cooper’s ligaments and the circummammary support, so those structures can spread and form a natural nest around the implant.

Why that matters:

* More anatomic support and stability
* Often allows beautiful upper-inner fullness with a more moderate implant
* A more refined, natural look that’s designed to hold up over time (in the right candidate)

Same neighborhood. Completely different architecture—and a very different philosophy about how the implant is supported.

In a traditional subglandular pocket, you separate the breast tissue off the underlying fascia, and the supportive ligaments end up sitting on top of the implant. In Preservé, we work within the posterior layer of the breast itself, hydrating and expanding it to create the pocket while preserving Cooper’s ligaments, so those ligaments can spread and “nest” the implant for better anatomic support.

Same neighborhood. Completely different architecture.
