Preservé® may be a great option for breast augmentation—but being interested in the procedure doesn’t necessarily mean you’re a candidate for it.

The right operation should be determined by your anatomy, tissue characteristics, existing implants, and the result you’re trying to achieve—not by the name of a technique.

For some women, that may be Preservé®. For others, a traditional breast augmentation, breast lift, or even no surgery at all may be the more appropriate recommendation.

That’s the purpose of a real consultation: finding the right procedure for the patient, not finding a patient for the procedure.

What do we actually know about Preservé®—and what are we still learning?

That distinction matters.

Breast augmentation decisions should be based on evidence, surgical principles, and an honest understanding of the limitations of the available data—not simply because a procedure is new or heavily discussed online.

Motiva® implants have been studied through a U.S. FDA clinical trial, while many of the surgical principles behind Preservé®, including prepectoral implant placement and tissue-preserving techniques, build upon established concepts in breast surgery.

But Preservé® as a specific branded technique is newer, and long-term technique-specific published evidence is still developing.

Patients deserve to know the difference.

When researching any procedure, ask your surgeon what evidence supports their claims, how much of their opinion comes from their own clinical experience, and what questions science hasn’t answered yet.

Innovation never stops—and neither does education.

I’m visiting the Victoria Clinic in Stockholm, Sweden, where breast preservation surgery was pioneered, to learn about the latest advancements in minimally invasive Preservé™ breast augmentation.

Although I’m proud to be one of the busiest Preservé trainers in the United States, remaining connected with leading international colleagues is essential. The best surgeons never stop learning, refining their techniques, and bringing the safest, most advanced options home to their patients.

My goal is simple: to continue elevating the standard of breast augmentation and provide my patients with the most current techniques, natural-looking results, and rapid-recovery experience possible.

A successful breast augmentation recovery isn’t just about what happens in the operating room—what you do afterward matters too.

One of the most important parts of recovery is allowing your body time to heal without unnecessarily increasing the risk of postoperative bleeding or other complications.

And “taking it easy” applies to more than just your chest. A strenuous leg workout can still significantly raise your blood pressure, which is why restrictions on exercise may include both upper and lower body activity during the early recovery period.

Avoid smoking and va**ng, respect your activity restrictions, and follow the postoperative protocol provided by your surgeon. Your specific restrictions and timeline should always come from the surgeon responsible for your care.

This is a Wide Awake Preservé® breast augmentation—before and immediately after surgery.

One important thing to understand: immediately after Preservé® is not the final result.

During the procedure, fluid containing local anesthetic is infiltrated into the breast to numb the area and hydrate the posterior lamella. That additional fluid temporarily increases breast volume and can make the breasts appear higher, fuller, and less natural immediately after surgery.

As that fluid resolves and the tissues begin to relax over the following days, weeks, and months, the breasts progressively soften and settle toward their final shape.

And “wide awake” doesn’t necessarily mean the same thing for every patient. Depending on the patient and what is appropriate for them, Dr. Epstein can perform Preservé® with local anesthesia alone or with additional medication/anesthesia support when needed. An anesthesiologist is available as part of the surgical team.

Not every patient is an ideal candidate for a Wide Awake Preservé® breast augmentation. Anatomy, the planned procedure, patient comfort, medical history, and other factors all play a role in determining the appropriate surgical and anesthesia approach.

A brand name doesn’t perform your surgery. Your surgeon does.

As Preservé® becomes more widely discussed, it’s important for patients to understand what actually matters when evaluating a branded procedure.

Technology and technique can provide surgeons with new tools—but they don’t replace surgical judgment, proper training, experience, or accountability.

That’s why Dr. Epstein encourages patients to look beyond the marketing. Ask your surgeon how frequently they perform the procedure. Ask about their training. Ask how they handle complications and revisions. And most importantly, understand who will be responsible for your care if something doesn’t go according to plan.

Choose your surgeon first. Then discuss the technique.

Have you ever wondered what it’s actually like to have a breast augmentation while fully awake—with no sedation?

This patient’s reaction says a lot.

With Dr. Epstein’s wide-awake Preservé® approach, appropriate patients may choose to have their procedure performed using local anesthesia alone. Other anesthesia and medication options are available based on the individual patient’s needs and preferences, and an anesthesiologist is present should additional medication or anesthesia become necessary.

While every patient’s experience is different, many of our wide-awake patients report being comfortable throughout their procedure and are surprised by how manageable the experience is.
