Ask someone if they’ve had their thyroid checked and the answer is usually yes.

But pull up the labs and sometimes you’ll find one number:

TSH.

TSH is an important screening test. But when symptoms are still there, it may not tell us the whole story.

Depending on your history and symptoms, a more complete thyroid evaluation may include:

→ TSH
→ Free T4
→ Free T3
→ TPO antibodies
→ Thyroglobulin antibodies

And here’s the part that often gets missed:

“Within range” doesn’t always explain why you feel the way you do.

Fatigue, brain fog, hair changes, feeling cold, constipation and difficulty managing weight can have many causes. Thyroid dysfunction is one of them.

That’s why we don’t assume every symptom is thyroid.

We look at the whole pattern.

There’s a difference between screening for thyroid disease and investigating why you still don’t feel well.

Sometimes that means looking beyond TSH.

The weight that suddenly settled around your middle.

Your jeans fit differently. Your waist is thicker. And the things that used to work… aren’t working anymore.

So you eat less. Add more cardio. Cut the carbs again.

But sometimes the problem isn’t that you’re doing less.

Your physiology changed.

Testosterone, insulin resistance, sleep, thyroid function, stress and loss of muscle can all play a role in changing body composition as men age.

And it can become a frustrating cycle: more visceral fat is associated with lower testosterone, while lower testosterone can make maintaining muscle and healthy body composition harder.

That doesn’t mean every man with belly fat needs testosterone.

Three patients this week. Three "normal" thyroid panels. All three telling me some version of the same thing — moving through mud, running on empty, not sick enough to explain it, but not okay either.

None of them were wrong. And their labs weren't wrong either — that's the part worth sitting with. A single number was never built to capture the whole thyroid picture. It tells you where a value landed on the day you drew blood, not how your body is actually converting and using that hormone hour to hour. Two people can land in the exact same "normal" range and feel completely different.

That gap — labs that look fine, a body that doesn't feel fine — is exactly where we start looking closer instead of shrugging it off.

You’re doing the work. So why isn’t your body responding like it used to?

You’re lifting weights.
Eating more protein.
Staying consistent.

But building muscle feels harder. Recovery takes longer. And your body composition just isn’t changing the way you expected.

You may not be doing anything wrong.

Testosterone matters for women, too.

Women have much lower levels than men, but androgens still play a role in muscle, strength, bone health, sexual function, and overall well-being. And androgen levels change as we age.

That doesn’t mean low testosterone is automatically the answer.

But if you’re putting in the work and something feels different, it may be worth looking at the bigger hormonal and metabolic picture instead of simply exercising more and eating less.

Same effort. Smaller return.

Sometimes the answer isn’t to work harder. It’s to figure out what changed.

PMDD: When Your Brain Is More Sensitive to Normal Hormone Shifts

For a week or two every month, you feel like a different person.

More anxious. Irritable. Overwhelmed. Maybe angry, hopeless, or completely unlike yourself.

Then your period starts.

And the fog lifts.

If this happens at the same point in your cycle month after month, it may be more than PMS. It could be PMDD.

Here’s what’s interesting: PMDD doesn’t necessarily mean your hormone levels are abnormal.

Research suggests the brain may respond differently to **normal hormonal fluctuations**, including changes in allopregnanolone, a progesterone-derived neurosteroid that interacts with the brain’s GABA system.

That can create a predictable pattern of anxiety, irritability, depression, overwhelm and mood changes that improves after menstruation begins.

That timing matters.

A “normal” hormone panel doesn’t necessarily explain away what you’re experiencing. Sometimes the pattern across your cycle tells us more than a single hormone level.

If this sounds familiar, track your mood and symptoms alongside your cycle for the next two months.

You may be surprised by how consistent the pattern is.

In perimenopause your estrogen isn't just running low. It's erratic.

For years the story we're told is decline. Estrogen slowly fades, symptoms slowly build, and one day you're in menopause. Clean. Linear. And not at all how it actually works for most women.

What really happens is closer to turbulence. Estrogen can spike and then crash within days. So the same woman, in the same month, can experience the heavy, bloated, tender symptoms of too much estrogen, followed by the hot, dry, achy symptoms of too little.

If that's you, there's nothing wrong with your read on your own body. The problem is that most care treats perimenopause like a steady slope, when it's really a series of peaks and drops.

This is why a one-time lab drawn on a random Tuesday can miss the whole story, and why "just take estrogen" or "just wait it out" both fall short. A hormone that moves needs a plan that accounts for the movement, not a single snapshot.

You're not too complicated to treat. You've just been treated too simply.
