C-SECTION BIRTH: MAJOR SURGERY, MAJOR LOVE 🤱🏻🔪 1 in 3 babies are born this way. A belly birth IS birth. This is power, not failure 👑✨

Cesarean Section = baby delivered through incisions in mom’s abdomen + uterus. Planned or emergency, scheduled or unplanned - all C-sections are life-saving and brave. Let’s demystify the 4 key steps 💪🏻

1. INITIAL ABDOMINAL INCISION: THE OR PREP 🏥
What you see: Surgeon + scrub nurse in OR. Mom draped, prepped with antiseptic. Monitoring station tracks vitals. OR lights on.
What’s happening:
- Anesthesia: Spinal or epidural = awake but numb waist-down. General = asleep, rare/emergency only
- Skin prep: Belly cleaned with antiseptic (yellow iodine). Sterile drapes placed
- Incision #1 - Skin: Usually “bikini cut” low transverse, 10-15cm, just above p***c hairline. Hidden in underwear line
- Layers opened: Skin → Fat/Fascia → Muscle Layer (abs are separated, not cut) → Peritoneum → Uterus
What you feel: Pressure, tugging, no pain. OR is cold. Curtain up so you don’t see. Partner can be by your head 🤍
Timeline: Prep 15-30 min. Actual incision to baby = 5-10 min in routine case.

2. FETAL DELIVERY: BABY EARTHSIDE 👶🏻
What you see: Uterine incision made. Baby still inside with umbilical cord + placenta in situ. Surgeon ready to deliver.
What’s happening:
- Incision #2 - Uterus: Low transverse cut in lower uterine segment. Less bleeding, stronger scar for future VBAC
- Baby delivered: Head or feet brought out first. Provider applies fundal pressure to help. Suction mouth/nose
- “Gentle C-section”: Curtain lowered, baby brought to chest immediately. Delayed cord clamping if possible
- Cord clamped + cut: Baby handed to pediatric team or placed skin-to-skin
What you feel: INTENSE pressure like an elephant sitting on chest. No pain. Nausea is common from meds/tugging. Then you hear the cry 😭❤️
Timeline: Baby out in 60-90 seconds from uterine incision. Fastest part of surgery.

3. PLACENTAL REMOVAL: THE SECOND BIRTH 🌸
What you see: Uterus open, placenta + membranes being removed with surgical tool. Umbilical cord clamped. Myometrium (muscle tissue) visible.
What’s happening:
- Placenta manually removed: OB checks it’s complete - retained pieces = infection/hemorrhage risk

- Uterine massage + Pitocin IV: Makes uterus contract hard to stop bleeding. Critical step
- Cavity cleaned: Endometrium checked. Bleeding controlled
What you feel: More pressure, cramping as Pitocin hits. Shaking is normal - adrenaline + meds + hormones
Timeline: 5-10 min. Slower than baby delivery because hemostasis matters.

4. CLOSURE & SUTURING: LAYER BY LAYER 🪡
What you see: Closed uterus with internal stitches. Abdominal wall + skin closure with forceps and sutures.
What’s happening:
- Layer 1 - Uterus: Closed with 2 layers of dissolvable stitches. Strong scar forms over 6 weeks
- Layer 2 - Peritoneum: Sometimes closed, sometimes left open - both safe
- Layer 3 - Muscle/Fascia: Abs brought back together + stitched. This layer gives strength
- Layer 4 - Fat: Closed if >2cm thick
- Layer 5 - Skin: Staples, sutures, or glue. Staples out Day 3-5. Glue peels off 2 weeks
What you feel: Tugging again. OR is quieter. You’re meeting baby, doing skin-to-skin if stable 🤱🏻
Timeline: Closure takes 30-45 min. Total surgery = 45-60 min routine, 90+ min if complex.

RECOVERY TRUTHS FOR C-SECTION MAMAS 💝
- Pain: Incision pain + gas pain + uterine cramps. Meds are your friend. Take them 💊
- Movement: Walk within 12-24hrs. Prevents clots, helps gas, speeds healing
- Lifting limit: Nothing heavier than baby for 6 weeks. No driving 2 weeks
- Scar care: Keep dry. Look for infection: redness, pus, fever >100.4°F, opening
- Emotions: Guilt, grief, or relief - all valid. Birth trauma is real. You still birthed your baby
- Future births: VBAC possible for many. Talk to provider about uterine scar type

C-SECTION REASONS: PLANNED OR EMERGENCY 🚨
Planned: Breech, placenta previa, multiples, prior C-section, maternal request, large baby
Emergency: Fetal distress, labor not progressing, cord prolapse, placental abruption, preeclampsia
Both are valid. Both are birth. Both save lives 💙
