¿Erecciones dolorosas, curvatura y disfunción eréctil? Podría ser la enfermedad de Peyronie.
Una curvatura nueva, dolor cuando tienes una erección… Quizás también notas que tus erecciones ya no son tan firmes como antes.

¿Podrían estar relacionadas estas cosas? Sí.

La enfermedad de Peyronie implica tejido cicatricial en el pene y puede causar curvatura, hendiduras y dolor, especialmente durante la erección y en las primeras etapas de la enfermedad.

Algunos hombres con Peyronie también tienen disfunción eréctil. Pero no toda erección dolorosa ni toda curvatura es enfermedad de Peyronie. Por eso necesitas una evaluación.
Si la forma de tu erección ha cambiado, especialmente junto con dolor o disfunción eréctil, no te quedes simplemente observándola durante un año entero.

¿Implante defectuoso? Hay solución.

Acabamos de operar a un paciente que llevaba años con un implante colocado en otro centro.

Los cilindros eran demasiado cortos: llegaban aproximadamente a tres cuartas partes del pene y no proporcionaban suficiente soporte al glande.

Esto le dificultaba tener relaciones s*xuales, pero, según nos contó, nadie le había explicado el problema.

Durante la cirugía de revisión, reemplazamos el implante por un AMS 700 con cilindros aproximadamente dos pulgadas más largos, adecuados a su anatomía.

También corregimos la posición de los tubos, reubicamos la bomba en un lugar más accesible del escroto y cambiamos el reservorio.

Es importante aclararlo: esas dos pulgadas corresponden a la diferencia de longitud entre los implantes; no significan necesariamente un aumento de dos pulgadas en la longitud del pene.

El objetivo fue corregir el soporte y mejorar su función. El resultado definitivo se evaluará después de la recuperación.

Si tu implante no funciona bien o sientes que algo no está bien, busca una segunda opinión.

Podemos examinarte, evaluar qué ocurre y explicarte las opciones para que tu implante funcione como debería.

YO NO PROMETO CAMBIOS DE TAMAÑO.

Si un cirujano te promete que después del procedimiento vas a medir exactamente esto o vas a ganar tantos centímetros, esa es una promesa muy peligrosa.

Yo no les hago esa promesa a mis pacientes porque ese no es el objetivo del procedimiento.

El objetivo del procedimiento es tener un pene firme con una buena erección.

Antes de la cirugía hacemos medidas, luego, después de colocar el implante, volvemos a medir de nuevo y ponemos el implante más grande que cabe.

Y sí, a veces vemos un poco más de longitud, un poco más de grosor y, en ocasiones, ambos. En la mayoría de los casos sí vemos un poquito más de largo el implante.

Pero nuevamente, el único objetivo del procedimiento es restaurar la función y darle al paciente un resultado confiable con una buena erección y un buen implante.

Si vemos algún cambio adicional en las medidas, perfecto, pero nunca voy a prometerles antes de la cirugía algo que no les puedo garantizar.

A person like this that has been suffering from erectile dysfunction really affects the relationship, affects self-esteem. They try every treatment. I mean, a lot of these guys, they try all the pills, then they go off to these clinics and they get these painful shots, and then they pay a lot of money for all these P-shots and lithotripsy for the p***s, and they try all these treatments that don’t work.

When in 15 minutes he’s going to have a reliable, efficient er****on. He’s going to be able to have a relationship with his partner again. And his life is going to come back to him just with a 15-minute outpatient surgical procedure.

And he’s never going to have erectile dysfunction again. He’s always going to function.

So always keep that option in the back of your mind. Don’t let it be the last option that you seek. It should be one of the first treatments that you seek once the pills stop working.

¿Cuáles son las posibles complicaciones de un implante peneano?

Un implante peneano sigue siendo una cirugía, así que sí, pueden existir complicaciones.

Una de las que siempre tomamos muy en serio es la infección. Por eso, durante la cirugía utilizamos técnicas especiales para prevenirla.

Usamos la técnica no-touch, cambiamos los guantes durante el procedimiento y tomamos múltiples precauciones para minimizar el riesgo de infección, incluyendo abundante irrigación con antibióticos.

Esa es una parte de la historia: la experiencia de tu cirujano, la técnica quirúrgica y todo lo que hacemos en el quirófano.

Pero hay una segunda parte: tú.

¿Cómo te estás cuidando después de la cirugía?
¿Estás cuidando la incisión?
¿Estás siguiendo las instrucciones postoperatorias?

Y si tu cirujano te dice que todavía no tengas relaciones s*xuales, por favor no decidas que tu nuevo dispositivo necesita una “prueba de manejo”. 😄

Sé que estás emocionado, lo entiendo, pero dale tiempo a tu cuerpo para sanar correctamente.

El cirujano tiene que hacer su trabajo en el quirófano, y después tú tienes que hacer el tuyo en casa.

Ambas partes son muy importantes para obtener el mejor resultado posible.

Y puedo decirte que tengo uno de los índices de infección más bajos en el mundo en mis casos de implantes peneanos.

If you’re taking Ozempic, Mounjaro, Wegovy, or another GLP-1 medication, your surgeon and anesthesia team need to know well before surgery.

These medications can slow down how quickly your stomach empties. Under anesthesia, food remaining in the stomach can increase the risk of aspiration, so it’s important for anesthesia to know and plan accordingly.

We don’t routinely ask every patient to stop GLP-1 medications anymore, but your surgical team absolutely needs to know that you’re taking them.

There’s another reason I pay attention to GLP-1s around surgery: recovery.

Your body needs calories, protein, vitamins, and nutrients to heal. If your appetite is significantly decreased and you’re barely eating, you may not be giving your body everything it needs. Protein shakes can be a helpful way to support your intake during recovery.

And one more thing patients don’t always realize: GLP-1 medications may affect libido in some people. If your s*xual desire changes while taking one, it’s worth discussing with your doctor.

What are the possible complications?

A pe**le implant is still a surgery, so yes, complications are possible.

One of the complications we always take very serious is infection prevention. That’s why during surgery, we use special infection prevention techniques.

I utilize the no-touch technique. We change gloves frequently throughout the procedure. We use a lot of antibiotic irrigations and take multiple precautions to minimize the risk of infection.

That’s one part of the story: your surgeon’s experience, surgical technique, and what we do in the operating room.

But there’s a second part to the story: you.

How well are you taking care of yourself before surgery? Are you taking care of the incision? Are you following your postoperative recommendations?

Don’t use the implant too soon, and if your surgeon tells you don’t have s*x yet, please don’t decide that your new device needs a new test drive.

I know you’re excited, I understand, but give your body time to heal properly.

The surgeon has to do his part in the operating room, and then you have to do your job at home. Both parts matter if you want the best possible result.

And be assured, I have one of the lowest infection rates in the world in my implant cases.

MY FRIEND HAS THE SAME THING…

No. Your friend has his problem. You have yours.

I can’t tell you how many times I’ve heard this from patients.

“My friend had the same thing.”
“He took this medication.”
“He told me what I should do.”

And then we actually see the patient and realize — it’s not the same thing at all.

Similar symptoms don’t always mean the same diagnosis. And trying to treat yourself based on somebody else’s experience can sometimes make things worse.

So please, don’t get your diagnosis from your friend — unless your friend happens to be your urologist. 😄

Go see a real doctor and find out what is actually going on with you. 👨‍⚕️

A pe**le implant replacement usually takes a little longer than the original surgery.

And sometimes I have a patient who comes to me with an implant that I didn’t put in, or even a different brand.
In those cases, I don’t always know exactly what I’m going to find until I get in there, so the surgery may take a little more time.

But if it’s my patient, I know what I put in, I know how I put it in, and I’m replacing a Boston Scientific implant with another Boston Scientific implant, usually it’s pretty straightforward.

Disfunción eréctil: ¿qué hacer?

El hombre, por vergüenza, está evitando sus encuentros íntimos, pero también la mujer, en algunos casos, puede decir: “¿Qué le está pasando? ¿Por qué no me busca? ¿Tendrá otra?”

O sea, yo creo que entran muchos factores encima de la mesa y supongo que el tema de la comunicación es fundamental: hablar en pareja, ponerlo encima de la mesa, que el hombre no tenga vergüenza de compartir lo que le está pasando y acudir a un profesional.

En este caso, tú imagino que tienes tantísimos pacientes. Cuando llegan ahí a tu consulta, ¿qué es lo primero que les recomiendas?

Sí, normalmente se comienza con lo que llamamos los tratamientos fáciles, ¿no? Las pastillas. Y muchas veces, a excepción de chequear los niveles de las hormonas, no se hacen más estudios, porque los estudios para determinar la causa de la disfunción eréctil a veces son un poquito invasivos.

Y si uno va a dar una pastilla de todas maneras, entonces, en la primera consulta, obviamente se hace un examen físico, se ve si tienen riesgos cardiovasculares para mandarlos a un médico, ese tipo de cosas, y se receta una simple pastilla que, en la mayoría de los casos, resuelve los problemas.
