Providence Hospice & Palliative Care

Providence Hospice & Palliative Care When facing a terminal diagnosis, you are dealing with more than physical ailments. Our Indiana office is located at:
10700 Park Place
St. John, IN 46373

Providence Hospice knows this, and we’ve put together a team of compassionate, skilled professionals who will walk with you and your family through this journey. Providence Hospice and Palliative Services serves Northwest, Indiana (Lake and Porter Counties), the Chicago Suburbs located in Will, Cook and DuPage Counties. A team of Providence staff (nurses, aides, social workers, chaplain, massage

therapy and volunteers) serves our patients during these difficult times with dignity. What makes our team unique is the special sense of calling that informs our work. Their medical qualifications are remarkable, but what sets Providence Hospice and Palliative apart is the personal commitment we have to offer hope and help to others when people need it most.

Sometimes, the greatest gift we can offer is simply our presence.

Meet Michelle McCarthy, Volunteer Coordinator for Providence Hospice and At Home. Through her work, Michelle helps prepare volunteers to walk alongside patients and families with compassion and respect. Whether sharing stories over old photographs or sitting quiety at a bedside, volunteers provide comfort in moments that matter most.

Not fixing things, not talking, not filling silence with helpfulness. Just being there — in the right way, at the right moment, for whatever the person in front of you actually needs.

Advance Directive vs. POLST

Both documents support patient wishes, but they serve different roles:

Advance Directive: broader personal wishes and values

Advanced Directives: Healthcare Power of Attorney

This is someone you trust to speak for you if you cannot speak for yourself.

Choosing this person is an act of trust and love—someone who understands your values and will honor them.

🌿 WEEK 2: Understanding Your Options

“Every voice deserves to be understood.”

Living Will

A living will allows you to share your wishes about medical treatments you may or may not want, especially in serious illness.

It can guide decisions about:
-Life-sustaining treatments
-Comfort-focused care
-End-of-life preferences

A living will is a legal document that tells doctors how you want to be treated if you cannot make your own decisions about emergency treatment.

🩷 Why It Matters

When families are faced with medical decisions, emotions can feel overwhelming.

An advance directive can bring:
-Clarity in uncertain moments
-Comfort for loved ones
-Confidence that decisions reflect your wishes

What is an advance directive? How do I set one up? Learn how to decide what health care you would want to receive if you were unable to speak for yourself.

Myth vs Truth

❌ Myth: advanced directives are only for people who are very ill

💙 Truth: any adult can benefit from having their wishes documented. Planning ahead is an active care, for yourself and for those you love.

Understanding Advance Directives

🌿 WEEK 1: Understanding Advance Directives

“A gift of clarity for those you love.”

What is an Advance Directive?

An Advance Directive is a written guide that shares your wishes for medical care if you are ever unable to speak for yourself.
It helps ensure your voice is still heard—especially during uncertain or difficult moments.

It can:
-Share what matters most to you in care
-Guide your loved ones
-Support your care team in honoring your wishes
