A quick re-introduction 👋

There are quite a few new faces here, so I thought it was time to introduce myself again.

I’m Dr. Sean Delaney, a double board-certified facial plastic surgeon in Houston. My practice is focused exclusively on the face, with a particular emphasis on deep plane facelift and neck lift surgery, rhinoplasty, and facial rejuvenation.

What I care about most is pretty simple: natural results.

I don’t want my patients to look “done,” pulled, or like a different person. My goal is for you to still look like yourself—just refreshed, balanced, and confident.

Outside the operating room, I’m a husband, a very new dad, a dog person, and probably more detail-oriented than is healthy. Life looks a little different these days—and I wouldn’t have it any other way.

I genuinely love what I do, and I still spend a lot of my free time studying techniques, reviewing outcomes, and figuring out how to make what I do better.

For those of you who are new here, welcome. I use this page to share real patient results, explain how facial plastic surgery actually works, talk honestly about recovery, and hopefully make the process a little less intimidating.

What is the best age for a facelift?

There isn’t one magic number. Facelift timing depends much more on your anatomy—skin laxity, jowling, and changes in the face and neck—than on your chronological age.

It’s also a very personal decision. The right time is when the changes you’re seeing bother you enough that you’re ready to consider surgery.

The goal isn’t to have a facelift as early as possible or to wait as long as possible. It’s to choose the right procedure at the right time for you.

Hair preservation during facelift surgery starts with the details.

When an incision is placed near the hairline, I’m thinking not only about where the scar will ultimately sit, but also about protecting the surrounding hair follicles.

Four surgical principles I pay particular attention to:
• Beveling the incision to follow the angle of the follicles rather than cutting across them
• Respecting follicle direction, which changes throughout the temporal and hairline regions
• Minimizing tension on the skin closure by allowing the deeper layers to carry the load
• Using cautery judiciously near the incision to limit unnecessary thermal injury to surrounding follicles and tissue

Hair loss can still occur after surgery for several reasons, but careful surgical technique can help minimize avoidable injury to the follicles.

Small details matter—especially around the hairline.

Do you have to shave your hair for a facelift? ✂️

Yes—but probably much less than you think.

For my facelift patients, I trim only a narrow strip of hair around the portions of the incision that extend into the hairline. This gives me the visibility and access I need during surgery while preserving as much of the surrounding hair as possible.

Because only a small amount is trimmed, the longer surrounding hair can usually be styled to conceal the shaved areas during recovery.

It’s a small detail, but thoughtful incision placement and hair preservation are both important parts of facelift planning.

One of the most common questions I hear after surgery is:
“Is this normal?”

The truth is that every surgical incision follows a predictable healing process. It may become redder before it fades, firmer before it softens, and continue changing long after the skin has closed.

Most surgeons tell patients when they’ll be presentable. I think it’s just as important to explain why their incision changes over the months that follow.

One of the most common misconceptions after surgery is that recovery means staying in bed.

While rest is an important part of healing, gentle movement is too.

After an isolated facelift, the risk of developing a blood clot is fortunately very low—much lower than after many body contouring procedures. But “low risk” doesn’t mean “no risk,” which is why we encourage our patients to begin taking short walks the day of surgery.

Walking isn’t about burning calories or getting exercise. It’s about keeping your blood circulating, helping your lungs recover after anesthesia, reducing stiffness, and supporting your body’s natural healing process.

Our recommendation is simple:
🚶 Walk for 5–10 minutes every 1–2 hours while you’re awake.
🦶 Perform ankle pumps while you’re resting.
💧 Stay hydrated.
😴 Then get back to resting.

Recovery isn’t about doing more—it’s about doing the right things consistently.

As always, follow your surgeon’s specific postoperative instructions, since every patient and every procedure is different.

Not every neck can look the same after surgery—and that’s an important conversation to have.

One of the most common questions I hear is:
“Can you give me a sharp, sculpted jawline?”

The answer depends on much more than the facelift itself.

Whenever possible, I encourage my patients to reach a healthy, stable goal weight before surgery. While we can remove fat from beneath the chin and directly contour the neck during surgery, we cannot change a person’s underlying anatomy or completely eliminate thicker soft tissues. Patients with a higher BMI or naturally heavier necks can still achieve beautiful, meaningful improvements—but the final result may not be as sharply defined as someone with thinner tissues.

This lovely patient is only 3 months after facial rejuvenation surgery that included a temporal brow lift, extended deep plane face and neck lift and buccal fat reduction, and I think her result is a wonderful example of realistic expectations. Her jawline is more defined, her neck is smoother, and she still looks completely like herself—just more refreshed. As swelling continues to resolve over the coming months, her contour should continue to improve.

Could we have done more? Possibly.

Facial aging or disproportion is rarely caused by a single problem, and there are often additional procedures that could further enhance a patient’s overall appearance. But my job isn’t to perform every procedure that’s available—it’s to develop a treatment plan that aligns with each patient’s goals, priorities, comfort level, recovery, and budget. The best surgical plan is an individualized one, not the longest one.

Beautiful surgery isn’t about making everyone look the same. It’s about helping each patient become the best version of themselves.

6 months after a complex revision rhinoplasty.

When she came to see us, it had been more than two years since her original rhinoplasty performed elsewhere.

She was left with:
• Asymmetric nostrils
• Right nostril retraction
• Left nostril collapse with breathing concerns

Despite three in-office revision procedures, she was ultimately told that this was “as good as it was going to get.”

Revision rhinoplasty is among the most challenging operations in facial plastic surgery. Every case requires rebuilding support, working through scar tissue, and correcting problems without creating new ones. It’s never a surgery we recommend lightly.

Our goal wasn’t to create a different nose—it was to restore balance, structural support, and a natural appearance.

To accomplish this, we reconstructed the framework using:
• Composite ear cartilage graft to lower the retracted right nostril
• Extended unilateral spreader graft
• Septal extension graft for stable tip support
• Lateral crural strut graft to reinforce the collapsed left nostril
• Mastoid periosteum graft to improve tip projection and soften the contour

Now, at 6 months, her swelling continues to improve, while her nostril symmetry, tip support, and overall contour are dramatically improved.
