That blue-grey shadow after under-eye filler isn’t a bruise, and it usually doesn’t fade on its own.

It’s called the Tyndall effect — light scattering through filler that’s sitting a little too shallow. The thin skin under the eye makes it especially visible.

The instinct is to add more filler to cover it.

Get it looked at first — sometimes the answer is dissolving a small amount, not adding anything.

Usually routine. Until it isn’t.

This patient came to PH MED for a second opinion after visiting a local ER. Their filler had been injected elsewhere, in and around the nose, using an unknown product type.

This was a serious vascular occlusion—a blockage of blood flow that put the patient at risk of skin and tissue loss. The affected area extended up the bridge of the nose toward the forehead. Fortunately, no vision loss was reported.

PH MED became involved to provide a focused assessment using ultrasound and to evaluate the affected area and blood flow. Our priority was determining whether the threatened tissue was receiving adequate circulation and guiding further care. The amount of filler dissolved alone cannot confirm that blood flow has recovered.

Before any filler appointment:

• Know who is injecting you. Avoid unlicensed injectors, including anyone falsely presenting themselves as a physician. Verify a claimed physician license directly through the Medical Board.

• Ask for the exact product brand, type and manufacturer.
• Keep a photo or copy of the packaging and lot number, plus a written record of the amount injected.
• Ask how your injector recognizes and manages complications.

Unusual or worsening pain, pale or dusky skin, or spreading discoloration after filler requires immediate medical assessment. Any vision change is an emergency—seek emergency care immediately.
