Check out this video snippet taken in the OR as Dr. Granzow describes the process during a vascularized lymph node transfer (VLNT) surgery. Warning: The short clip may be a bit graphic for some.

Here we're harvesting out a lymph node transfer flap so there are some bigger lymph nodes that we've left intact which we want to preserve, and there'll be some up w/ us in the flap here.

Here's a vein that's crossing up above. And then we can see very carefully if we look underneath here, there's an artery and the artery is pulsing right there, and then here's another vein that's going w/ the artery traveling up in here, and if we look carefully we can see the artery pulses here, pulses up in here and pulses up in there.

And this is how we preserve the blood supply and make this function when we bring it up to the axilla or armpit.

Today our CLT Marcy highlights some of the aids people living with lymphedema may require:

A lot of times, patients who are just learning about lymphedema are given very little instruction and coaching regarding the management of their condition. Often, they are simply told to "start wearing compression". Often, additional devices are required to help patients optimize their compression. Here are some items one may need to use at some point in their lymphedema management routine.
1. Arion Easy-Slide- this device is slippery and helps people put their leg and arm garments on much easier.
2. Donning gloves- these are the 2 main designs; "dishwashing" and "gardening" styles. Both enhance the ability to grip compression material easier for better positioning.
3. Komprex foam- this dense "orange" foam can be used under the garment (against the skin) to provide additional compression to areas that may be thicker or holding more fluid. Care should be used to not position over bony prominences or areas where the tendons are close to the surface to avoid causing an issue. This foam can also be cut into shapes to contour around trouble areas. You can see the smaller piece has been trimmed to fit around the back of an ankle.
4. Komprex ii foam- a.k.a. "channel foam" or "noodle foam", this material is worn between the garment and the skin in areas that are thicker and need more texture to "decongest" the region. It is important to make sure to not position this foam in a horizontal or semi-horizontal position as it can create a barrier to lymphatic flow.

While these devices can be helpful in the proper application of garments and to spot-treat congested areas, it is important to discuss the use of these devices with your Certified Lymphedema Therapist (CLT) and ensure they provide the proper education to avoid potential issues with their use.

Here is a quick view of Dr. Granzow as he performs an intraoperative ultrasound on the patient prior to an LVA bypass surgery (lymphaticovenous anastomosis). This ultrasound can also currently be performed in our office.

So we find the veins here using an ultrasound. This helps guide our placement of our incisions and lets us make better connections during our anastamoses.

How long have you been a Paralegal?
I have been a paralegal for over 20 years. Before joining Team Granzow, I worked primarily as an in-house paralegal for a homebuilding company.

What is your role and how long have you worked at Granzow Lymphedema & Lipedema Center?
I have worked at Granzow Lymphedema & Lipedema Center for almost six years as a healthcare paralegal with our incredible appeals team: Amy and Andrea.

What do you like about working at GLLC?
I love working with our patients. I am inspired by the grit and optimism they display despite the setbacks many of them encounter with their insurance companies, as they fight to get their lymphedema surgeries approved.

Fun Fact:
I am a mom to teenage twins as well as a few fur babies who keep me very busy.
When I have free time, I love going to see musicals and paddle boarding.

Bianca and Sam took this video for us. It demonstrates how an LVA (lymphaticovenous anastomosis) site functions in relation to lymphatic drainage and vein clearance. The process is outlined as follows:

* Dr. Granzow identifies the LVA site on the screen, showing the lymphatic system draining directly into a vein;

* When pressure is applied to the site, lymphatic fluid drains from the lymphatic vessels into the vein; and

* Dr. Granzow notes that performing this action effectively flushes the vein.

Ever wonder what the aspirate from your Granzow SAPL℠ (Suction Assisted Protein Lipectomy) surgery might look like? Here's an example and a couple interesting facts: The average volume aspirated is almost 4 liters for legs (equivalent to two 2-liter soda bottles). The aspirate consists of proteins and fats. It is much tougher and irregular than, for example, fat aspirated from cosmetic liposuction. Sorry, it's a little noisy....

With Lindsey's assistance, today we showcase one of our own: Samantha Teeter. Sam has been one of our Physician Assistants for 4 years.

On Becoming a Physician Assistant:
Since earning my Physician Assistant degree in 2019 and transitioning from Family Medicine to Granzow Medical in 2022, I’ve continually seen the vital impact PAs have on healthcare teams through patient care and lasting relationships with patients and colleagues.

Team Granzow Highlights:
My favorite parts of being a PA on Team Granzow include:
- Schedule variety consisting of work in the office, OR, and hospital. No week is exactly the same as the next!
- I enjoy working on a team of highly motivated and knowledgeable professionals collaborating to provide the best care for our patients.
