The idea for Tria Health came from a couple of things we already knew to be true. First, an incredibly high number of people don’t use their medication correctly, and that costs the system billions of dollars a year. Second, pharmacists play an incredibly important role in the health care system, and their expertise is a largely untapped resource. We began in 2009 as a pilot program designed to he
lp patients with diabetes – a group of people who often use several different medications to control their condition – and their employer reduce costs by providing working with clinical pharmacists to ensure their medicine was producing the best possible outcomes. Those involved in the pilot immediately recognized the value of providing this service, and we decided to expand our program to include plan members with other chronic conditions outside of diabetes. Tria Health focuses on resolving the high costs and health risks associated with the mismanagement of medication. Our pharmacists speak to patients one-on-one, taking the time to go through each medication in their regimen and making sure it’s safe and effective. They communicate any problems or recommendations to both the patient and their primary care physician. The name Tria Health came from that circle of care we facilitate. Tria – meaning three – represents the collaboration among our pharmacist, the patient and the patient’s physician that we believe provides the best outcomes for everyone. Since the beginning, Tri Health has provided personalized pharmacy guidance to help companies, ACOs and health plans reduce their medical costs and improve their patient outcomes. We were founded on a commitment to better our patients’ health and ensure that their medications are safe, effective and affordable. And we’ll continue to do just that, even as conditions, medications and the world of health care change.
