Because of our continued growth, we are adding TWO NEW Care Coordinator positions to our team!

We are looking for compassionate, organized, dependable individuals who enjoy helping people and want to be part of a growing home health care company.
DOE 25.00 - 26.00 per hour

❤️ AS A CARE COORDINATOR, YOU WILL:

• Coordinate client care and caregiver schedules
• Communicate with clients, families and care staff
• Help ensure shifts are properly staffed
• Follow up on call-outs, schedule changes and client needs
• Maintain accurate client and employee documentation
• Support caregivers and help resolve day-to-day concerns
• Work closely with the Chinook office team
• Help ensure client care needs are being met
• Provide excellent customer service to our clients and their families

⭐ WE’RE LOOKING FOR SOMEONE WHO IS:

• Dependable and organized
• Compassionate and caring
• A strong communicator
• Comfortable multitasking in a busy environment
• Detail-oriented with excellent follow-through
• Comfortable using computers and learning new systems
• Able to work independently and as part of a team
• Experienced in healthcare, home care, caregiving, scheduling or office administration is a plus

Home care or healthcare experience is preferred, but we are willing to train the right person!
