Dashboards have changed the way healthcare organizations monitor performance, identify risk, and manage operations. But while metrics can highlight trends, they cannot always explain the story behind them.

A spike in denials, an increase in Level 5 visits, or a growing charge lag may appear actionable at first glance. The question is whether the data reflects a true issue, or whether the clinical context tells a different story.

The patient is doing well.

Sounds fine, right?
