Our Heart Failure Empowerment Program helps home health patients understand what to watch for, manage their condition with confidence and stay active in their care at home.

On World Hospice and Palliative Care Day, we're highlighting the important role hospice and palliative care play in helping patients and families navigate serious illness.

Both hospice and palliative care focus on comfort, quality of life and support, helping patients receive care that aligns with their goals and wishes. Palliative care can begin at diagnosis and alongside treatment. Hospice care is typically chosen when treatment is no longer working or a patient decides to stop curative treatment for a terminal illness, shifting the focus to comfort, quality of life and support for both the patient and their loved ones.

Ms. Theresa Sherer who is originally from Pineville, LA, she joined our team in May of 2024 where she brings both passion and heart to everything that she does. After earning her RN License in 2022, she quickly stepped into her role with confidence, compassion and a true calling for her patient care.

Known for her huge heart and genuine love for helping others, she consistently goes above and beyond to make a difference in the lives of those she serves. Her compassionate nature and positive attitude are felt by both the patients and her team members.
