Two tips re: complex patient presentations ➡️

1️⃣ More complex symptom presentation does not equal more complex intervention. In fact, most times, it’s the opposite - the more complex, the symptom presentation the more basic and foundational the intervention needs to be for a longer period of time.

2️⃣ Getting a good symptom timeline is always important, but for a complex presentation it’s IMPERATIVE that you get a good symptom timeline and you go back far enough to make sure you don’t miss relevant information

Whether you treat folks with bladder pain syndrome or other lower urinary tract symptoms, it’s always important to go back to the basics!
~
The bladder has pain referral sites to:
1️⃣ sacral apex
2️⃣ suprapubic region
3️⃣ groin/adductors
~
Its innnervation is:
➡️ Autonomic sympathetic: T10-11-L2, hypogastric nerve to cause relaxation of the detrusor and bladder filling
➡️ Autonomic parasympathetic: S2-4, pelvic splanchic nerves causing contraction of the detrusor and bladder emptying
➡️ Somatic via the pudendal nerve: S2-4, has baldder afferents and innervates the external urethral sphincter

The urachus is a remnant of the median umbilical ligament that connects the urinary bladder to the umbilicus and is continuous with the falciform ligament of the liver.
~
Its always so important to remember the viscerosomatic and somatovisceral reflexes as we are deciding how to best intervene and treat our patients with bladder symptoms or pain!

Being able to confidently say - based on your experience, your area of expertise, and the evaluation and observation that you’ve done - that something is “normal” and “nothing’s wrong” is SO valuable!!

So often, I think we - as providers -we pride ourselves on being able to tell somebody something that we can help with. That we can “fix”. Naming the dysfunction. Naming the issue.

And that is important. Dont get me wrong!

But it’s equally, if not more important to also be able to tell somebody - “everything is looking great” or “ things are progressing normally” or “ I know you think that that feels or it looks weird, but it’s actually totally normal.”

There is so much value in that. Dont’ ever forget it! 🩷
