A *technically* successful fine-needle aspirate doesn’t always produce a diagnostic sample…🔬

Easy fix. Before submitting feline splenic or liver cytology, I check a stained slide in-house to confirm that I collected representative cells (not just blood, fat, or debris)!

Choosing the right sampling technique is only part of the process! Idenifying the vessel, keeping an eye on your needle, and utilizing sedation and analgesia are all essential considerations when performing a feline splenic aspirate.

With careful patient selection, monitoring, and technique, splenic aspiration can be a relatively low-risk way to obtain valuable diagnostic information.

BEST LIVER FNA TIPS IN 1 MINUTE! LET’S GO! 👏

Map the vessels with color Doppler. Turn color flow off before sampling so the needle remains visible. Choose a controlled needle path. Keep ultrasound gel away from the cytology sample. Never lose sight of the needle while it’s within the liver...

Two big goals: safely collecting cells from the intended location, and giving the pathologist the best sample possible!

The best equipment for an ultrasound guided liver fine needle aspirate (FNA) is the equipment that allows you to clearly visualize your needle path and maintain precise control! 💉

Choose the tools that give you the safest route, the clearest visualization, and the greatest control for the patient in front of you.

One more tool to remember: cat-friendly SEDATION! 💜

Cytology can help identify certain conditions, including hepatic lipidosis and some discrete liver masses, but it can also help us decide what comes next! Do the findings fit the clinical picture? Is there evidence of an underlying disease process? Will we need histopathology and a liver biopsy to reach a definitive diagnosis?

In feline liver disease, consider FNA to narrow the differential list and help choose the most appropriate next step.

“Mostly normal” bloodwork may not tell the whole story…

In this patient, the only laboratory abnormality was an elevated ALT, but cats can have significant disease even when much of their bloodwork remains within normal limits!

So yes, that single elevation deserves investigation. This is where ultrasound becomes an essential tool in our diagnostic toolbox: helping us look beyond the numbers, rank our differentials and determine the most useful next step.
