Orthopedics This Week
The Spine Surgery ā€œAvengersā€ Just Assembled in Atlanta

Somewhere in Atlanta, a lumbar disc probably realized it had absolutely no chance.
At Emory Healthcare, Daniel Refai and team completed what is believed to be the world’s first continuously navigated endoscopic transforaminal lumbar interbody fusion (TLIF) — a sentence so packed with buzzwords it practically qualifies for CME credit by itself.
But beneath the alphabet soup is something genuinely important for spine surgeons: This may represent the next evolution of minimally invasive lumbar fusion.
The Procedure: Endoscopic TLIF Meets ā€œEyes-Onā€ Navigation
Traditionally, navigation in spine surgery tends to appear for key moments: Plan the incision, place the screws, maybe verify the cage, then everyone moves on with life.
This case was different.
Navigation reportedly stayed active through the entire operation: Incision planning, decompression, interbody prep, cage placement, and pedicle screw fixation.
Think less ā€œGPS when you need directionsā€ and more ā€œGoogle Maps glued to your windshield for the whole road trip.ā€

The workflow combined: Amplify Surgical, Inc.’s dualPortalĀ® and dualXĀ® systems, Stryker (now VB spine) navigation and endoscopy, and spine instrumentation.

In spine surgery terms, this was basically: ā€œWhat if we connected everything?ā€

Spine Surgery Is Becoming an Ecosystem

This wasn’t just a cool case report. It’s another sign that spine surgery is rapidly evolving from standalone implants to fully integrated digital surgical ecosystems.

The modern spine OR increasingly looks like navigation, robotics, endoscopy, powered instrumentation, AI-assisted planning, expandable implants, real-time imaging, and at least one rep silently updating software in the corner.

If this trend continues, future fellows may someday ask: ā€œWait…you used to place screws without continuous navigation?ā€

Much like today’s residents react to stories about paper charts and hand-written op notes.

Congratulations to Josha Woodward, MD, on completing your first dualPortal2.0 case at OHSU Health in Hillsboro, OR!

Please join us in congratulating Dr. Woodward on his accomplishments and dedication to providing the best care for his patients. Welcome to the growing Ā® endoscopic community!

The dualPortalĀ® Endoscopic Technique is a novel two-portal endoscopic approach to the spine that allows surgeons to easily learn and perform a wider array of lumbar spine procedures than the conventional one-portal technique.

dualPortalĀ® is a versatile and powerful technique in which the decoupling of the endoscopic camera from the surgical instruments allows for greater flexibility and enhanced visualization of spinal anatomy, applicable to a wide variety of lumbar pathology.

It also provides flexibility to perform endoscopic lumbar fusions with dualXSlim Dual Expanding Interbody Fusion System.

Becker's SPINE REVIEW

Daniel Refai, MD, of Atlanta-based Emory Healthcare, performed the world’s first continuously navigated endoscopic transforaminal lumbar interbody fusion.

Dr. Refai used Amplify Surgical’s dualPortal & dualX platform for the procedure, according to a May 17 news release. The case was also done in conjunction with Stryker’s Q Guidance System with Spine Guidance 5 with Copilot and VB Spine’s ES2 pedicle screws.

Pedicle screws were placed under power with continuous navigation. Amplify’s tools enabled an integrated endoscopic TLIF workflow, and the visualization systems gave real-time imaging.

ā€œThis case represents a meaningful leap forward in how we approach minimally invasive lumbar fusion,ā€ Dr. Refai said in the release. ā€œMaintaining continuous navigation throughout an endoscopic TLIF — from decompression through interbody placement and powered screw fixation — gives us a level of precision and confidence that has not previously been possible.ā€
