Pathways Providing Care Navigation, Hospice, and Bereavement services to the Greater Lakewood/Long Beach community - all at no cost - for nearly 40 years

A community-based 501 (c) 3 not-for-profit organization, Pathways has been a part of the Greater Lakewood/Long Beach area since 1985. Through our three-part “Circle of Care”, we service those facing some of life’s most difficult challenges by providing (1) Volunteer Care Navigation for the frail, older adults, and chronically ill, (2) Hospice Care for clients and their families experiencing a terminal illness, and (3) Bereavement Counseling and Support for children, adults, and families in grief. We do all of this at no cost, eliminating barriers to service for our clients and their families. Pathways has always been a ray of hope in a sea of confusion, heartache, and healthcare red tape.

This Thanksgiving, our Pathways team will be putting together holiday baskets for our clients! For many, this may be the only holiday cheer they receive this season. ❤️

❤️ A Glimpse Into Our Community ❤️

Over the last few weeks, our Pathways team has had the opportunity to get out and connect with our amazing community at several different events!

🏥 Cerritos Senior Center’s Annual Health & Wellness Fair — We had the opportunity to connect with more than 300 seniors, share information about Pathways, and learn more about the incredible resources available to support older adults and their families.

🚶 City of Lakewood’s Mosey with the Mayor — It was wonderful connecting with Lakewood residents, sharing more about Pathways and the support we provide, and building relationships with other local agencies and nonprofit organizations.

🍨 Senior Center Month at Weingart Senior Center — We celebrated Senior Center Month with ice cream sundaes and more than 50 participants! It was a great opportunity to spend time with local seniors and join in on the celebration.

We’re always grateful for opportunities to get out into the community, build meaningful connections, and share the Pathways mission with those we serve. ❤️

🎉 Pathways Back to School Drive is here! 🎉

Now that all of our kiddos are back in school, it means that Pathways school-based grief workshops are back too! 📚🎒

These amazing workshops provide students with a safe and supportive space to express their feelings, connect with others, and learn healthy ways to navigate grief and loss. ❤️

Our hands-on activities and creative materials give students opportunities to reflect, express themselves, connect with others, and explore healthy coping strategies.

To make this possible, WE NEED YOUR HELP! We are currently seeking donations of craft supplies and other materials to help us continue providing these workshops at no cost to schools or students.

Within our community, there are so many individuals and families that are struggling and desperately in need of help. They face challenges such as the hardships of aging, life-limiting illnesses, and the loss of precious loved ones.
