What makes a compound “legal” to make through a 503A compounding pharmacy?

It’s not a judgment call — it’s three specific rules.

Under federal law (Section 503A), we can only compound a bulk drug substance if:

(1) it complies with an official USP or NF monograph, or (2) no monograph exists, but it’s a component of an FDA-approved drug, or (3) neither applies, but the substance appears on the FDA’s 503A Bulks List after formal review.

If a substance doesn’t clear one of these three, we don’t compound it — no matter how popular it is on social media or if another pharmacy is offering it. This is the filter every ingredient in our formulary passes through before it ever reaches a patient.

You’ve probably seen the term “503A pharmacy” in a bio and scrolled past it. Here’s what it actually means.

A 503A pharmacy compounds medications for a specific, named patient based on a licensed provider’s prescription. We’re not mass-manufacturing a product to sell off a shelf — every preparation is made after a prescription exists, for the person it’s written for.

That distinction is the entire reason compounding exists: to give patients and providers options commercial manufacturing can’t — a different strength, a different delivery method, an ingredient a patient can’t tolerate, removed.

🔬 **Big news in the compounding world — but here’s the real story.**

Last week, the FDA’s Pharmacy Compounding Advisory Committee (PCAC) met to review seven peptides for potential inclusion on the 503A Bulk Drug Substances List — including names many of you have been asking about like BPC-157, TB-500, and Epitalon.

The committee voted to recommend six of the seven peptides for inclusion. That’s a meaningful signal — but here’s what it does NOT mean: it does not mean these peptides are approved for compounding yet.

📌 A PCAC recommendation is advisory only. The FDA still has to review the full record and make its own formal decision — a process that could take months, with rulemaking likely stretching into 2027. A second PCAC meeting on additional peptides is already expected in early 2027.

This was the first real step in a long regulatory process — not the finish line. At American Wellness Pharmacy, we’re watching this closely and will only ever compound what’s legally permitted under Section 503A, backed by a valid prescription and proper medical oversight.
