A provider has been collecting complaints. Their peer reviews come back at 100%, every time, no concerns flagged.

Misty Shell, MBA-HCM, LPN , VP of Clinical Strategy and Quality at Medplace, on the moment a quality lead realizes the numbers have stopped lining up, and why it usually isn't the provider, it's who's doing the reviewing.

Her phrase for the fix: reviewers who aren't sitting three desks down.

The providers who chase their peer review results aren't the ones scoring badly.

Sherrie Vivian, Director of Healthcare Quality at Franklin Primary Health Center: they're the B's. One provider calls her the moment results land, sets up a Teams meeting, and walks the charts with her until she understands exactly what she missed.

"She's engaged, and that's all I want."

Feedback clinicians chase beats feedback they file.

Health Resources and Services Administration (HRSA) doesn't tell you how many charts to review. It tells you to review every quarter, and to have a policy you actually follow.

Misty Shell, VP of Clinical Strategy and Quality at Medplace, on what Chapter 10 and your site visit handbook actually require, and what surveyors ask next: what do you do when you see a trend that doesn't align with your quality goals?

Medplace recommends at least five to six charts a quarter.

Their blood pressure numbers looked like a care problem. They were a documentation problem.

Controlling high blood pressure is a CMS quality measure and feeds payer star ratings. Sherrie Vivian's peer reviews at Franklin Primary Health Center. showed the second reading was being taken, just recorded on an EHR flow sheet that didn't count toward the measure.

IS team in, mapping changed, number moving.

That finding didn't come from a chart audit. It came from peer review.
