Low mood. Broken sleep. Brain fog. Fatigue. Irritability. Weight changes. Low libido.

That's the symptom list for depression. It's also the symptom list for perimenopause. Nearly identical — which is exactly why one gets mistaken for the other.

In a Harvard study that followed women aged 36–45 with no history of depression, those who entered perimenopause were about twice as likely to develop significant depressive symptoms. No prior history needed. The transition itself carries the risk.

And here's how it gets missed: a 15-minute visit, a screening questionnaire, four honest yeses. If nobody asks what your cycles have been doing for the last year, perimenopause never enters the conversation.

To be clear — depression is real and treatable, and if you're being treated for it, keep going. But this is also the season to have your hormones evaluated. If they're driving part of what you're feeling, no antidepressant is going to find that. Hormones are the part I look at.

There's no single answer there either. Estradiol, progesterone, and testosterone have each been studied for mood in this transition — which of them matter for you, if any, depends on your labs, your symptoms, and your history. No standard combination. No default starting point.

You're not imagining it. And you haven't become a less resilient person.
