🏡 Going Home From the Hospital? 5 Questions Families Should Ask

Hearing “You’re ready to go home” is usually welcome news.

But going home after a hospital stay can also be a big transition—especially when someone is weaker, moving differently, taking new medications, or simply not quite back to their usual routine.

Before discharge, here are 5 questions worth asking:

1️⃣ Is home safe for their current mobility?
Can they safely get to the bathroom, navigate stairs, get in and out of bed, and move around the home? A little extra assistance may help reduce the risk of falls.

2️⃣ Do they understand their medications?
Hospital stays can mean new medications, discontinued medications, or changes in timing and dosage. Make sure everyone understands the discharge medication list and knows who to contact with questions.

3️⃣ Are they eating and drinking enough?
Fatigue, weakness, pain, or a change in routine can make preparing meals and staying hydrated more difficult than expected.

4️⃣ Who is helping with follow-up care?

You start driving them to appointments.
