medication independence doesn’t magically happen at 18.

small habits like using a pillbox, keeping a med list in your phone, tying medications to an existing routine, and actually knowing what you take and why can make a huge difference.

at perfecting peds, we want adolescents to feel confident managing their own medications before they have to do it alone.

🌟 Patient Care Win

A 17-year-old patient came to us with severe withdrawal symptoms while tapering off venlafaxine ER.

Her initial taper used 50% dose reductions, which proved too aggressive. Because the extended-release formulation couldn’t simply be split in half, our pharmacist created a slower taper plan and helped cross-titrate her to sertraline.

The result: she successfully discontinued venlafaxine, transitioned to sertraline, and is no longer experiencing the previous side effects or severe withdrawal symptoms. 💙

Sometimes the biggest medication win isn’t starting something new. It’s helping a patient come off a medication safely and comfortably.

🔥 Unpopular opinion: tablets and capsules are underrated in pediatrics.

Liquid isn’t always easier.

💊 Some tablets are scored. Some can be crushed easily without a mortar and pestle. Some capsules can be opened and mixed into food.

✈️ Travel can be easier.
No bulky liquid bottles. No syringes. No refrigeration for certain medications. When appropriate, everything can fit neatly into a pill organizer.

🍫 Taste can be easier too.
Chocolate syrup. Applesauce. Pudding. Yogurt. Raspberry puree. The right vehicle can make a huge difference, as long as it’s compatible with the medication and there are no food-drug interactions.

👧 And yes, your child may be able to swallow pills.
A little practice, creativity, and support from Child Life can go a long way.

Of course, not every tablet can be crushed and not every capsule can be opened. Extended-release, enteric-coated, hazardous, and certain other medications need special consideration.

No matter how many medications we’ve seen, these three rules stay the same:

✨ Check the expiration date
💉 Oral syringe > household spoon. Every time.
🌡️ Store meds at the temperature on the bottle. Your car does not count as a medicine cabinet. 🚗☀️

Simple? Yes.
Important? Also yes.
