What I wish every patient knew before their first visit with an eye doctor

At your first visit, I’m going to ask you more than, “What brings you in today?”
I’ll ask:
— Have you had any eye surgeries or injuries?
— Have you had previous eye exams or imaging?
— What medications or supplements do you take?
— Does anyone in your family have glaucoma, diabetes, retinal disease, or other eye problems?
— When did you first notice a change in your vision, and has it changed over time?

These answers matter. Previous exams and imaging help me see changes over time. Your family history can point to conditions we should watch more closely. And your medications, surgeries, and past injuries can all be important when I evaluate your eyes.

If you have previous exam results, retinal images, a medication list, or records from past eye surgeries, bring them with you. I want to understand not just what your eyes look like today, but how they have changed over time.

На первом приеме я буду спрашивать вас спрашивать не только: «Что вас беспокоит?»

Я спрошу:
— Были ли у вас операции или травмы глаз?
— Какие обследования вы проходили раньше?
— Какие лекарства и добавки принимаете?
— Были ли у близких родственников глаукома, диабет, заболевания сетчатки или другие проблемы со зрением?
— Когда вы впервые заметили изменения и как они менялись?

Ответы на эти вопросы очень важны, ведь предыдущие исследования позволяют увидеть изменения со временем, семейная история помогает понять возможные риски, а информация об операциях, травмах и лекарствах влияет на то, как я оцениваю состояние глаз.

Если у вас сохранились старые обследования, снимки, список лекарств или документы о предыдущих операциях, возьмите их с собой. Мне важно видеть не только, что сейчас происходит с глазами, но и всю историю вашего зрения.

The best words an eye doctor can hear.

After several months of treatment for wet age-related macular degeneration, a patient told me: “Doctor, I can see my grandchildren’s faces again.”
I still remember those words.

We had been treating his retina with regular injections to control abnormal blood vessel growth and swelling. Treatment takes time, and regular follow-up is important.

Sometimes we can improve vision. Sometimes the goal is to preserve the vision a patient still has.

For a patient, the result is not just a number on an eye chart.
It’s being able to read a book again. Look at a photograph. See the expression on someone’s face across the room.

So when a patient tells me, “I can see my loved ones again,” I know the result means much more than better vision.
Those are the moments that stay with me as a doctor.

Самая приятная фраза, которую может услышать офтальмолог

Однажды после нескольких месяцев лечения пациент с влажной формой возрастной макулярной дегенерации сказал мне:
«Доктор, я снова вижу лица своих внуков».

Я помню эту фразу до сих пор.

Мы начали лечение с регулярных инъекций в глаз, которые помогают контролировать патологический рост сосудов и отек в сетчатке. Лечение требует времени и регулярного наблюдения. Иногда зрение удается улучшить, иногда главная цель - сохранить то, что человек еще видит.

Для пациента результат измеряется не только цифрами на таблице у офтальмолога. Это возможность снова прочитать книгу. Увидеть фотографию. Разглядеть выражение лица человека, который сидит напротив.

И когда пациент говорит мне: «Я снова вижу лица близких», я понимаю, что за результатом лечения стоит гораздо больше, чем просто хорошее зрение.
Это одна из самых дорогих для меня фраз в моей профессии.

“Doctor, I’ve started seeing floaters. But my vision is still clear. Why?”

Yes, this happens, and it’s a complaint we see quite often.

Floaters can look like small dots, lines, or little cobwebs that move when your eyes move. They often appear because of changes in the vitreous — the gel inside the eye. Your visual acuity may stay exactly the same.

During an exam, I always ask: When did you first notice them? Have you noticed more of them? Are you seeing flashes of light?

Pay attention to sudden changes. If you suddenly see many new floaters, especially together with flashes of light, a shadow, or a curtain-like effect in your vision, the retina should be examined as soon as possible. These symptoms can be a sign of a retinal tear or detachment.

If you’ve recently noticed new floaters or flashes, especially when looking at a bright background, it’s worth having your eyes checked. An exam can tell us whether there is a reason for concern and whether urgent treatment is needed to protect your vision.

«Доктор, у меня появились мушки перед глазами. Но вижу я нормально. Почему так?»

Да, такое действительно бывает и мы часто сталкиваемся с этой жалобой пациентов.

Мушки - точки, ниточки, маленькие «паутинки», которые двигаются вместе с глазами. Часто они появляются из-за изменений стекловидного тела - геля внутри глаза. Острота зрения при этом может не меняться.
На приеме я всегда спрашиваю: когда вы впервые это заметили? Их стало больше? Есть ли вспышки света?

Наблюдайте за ощущениями: если мушки появились внезапно и их стало много, особенно если появились вспышки, тень или ощущение занавеса перед глазом, нужно осмотреть сетчатку. Такие изменения могут быть признаком ее разрыва или отслойки.

Если у вас недавно появились новые мушки, вспышки, особенно, когда вы смотрите на свет, лучше просто проверить глаза. По итогам осмотра можно понять, есть ли причина для беспокойства и нужно ли прибегать к экстренным мерам по спасению зрения.
