Newsletter 7 – Moderate Weight Loss VS Quick Excessive Weight Loss
Moderate weight loss is healthy, but excessive weight loss is unhealthy. While losing excess weight is helpful for one’s overall health and metabolic function, excessive weight loss can be damaging and dangerous.
Currently, GLP-1 medications, and the latest addition to this group Retatrutide ( but not yet FDA approved) can cause a reduction of weight that approaches 30% of one’s prior weight. Retatrutide is a more powerful GLP-1 agent because it contains three incretin hormones that help control appetite, energy balance and metabolism; GLP-1,GIP, and Glucagon, whereas Ozempic has one incretin agent and Zepbound has two incretin agents. This excessive loss of muscle, fat and protein can damage the heart, liver, kidneys and brain. Another newly recognized problem with the GLP-1 medications is that some overweight and even normal weight individuals are using these agents to such an extreme that causes them to develop or exacerbate an eating disorder like anorexia nervosa. The power of the GLP-1s, as one anorexic patient put it, is that these agents are like an 'anorexic he**in that affects their brain chemistry’, initially helpful, but later for some eating disorder patients, they are hard to stop. Excessive weight loss makes an individual fragile, gaunt, weak and to some extent immune-compromised, and their health is harmed and not helped by such severe weight loss.
It is advised that overweight individuals consult with a responsible and experienced health care professional before and during their use of the GLP-1 medications. One suggestion that can help prevent protein and lean muscle loss and preserve their health is to eat enough high-quality protein, often recommended to be between 0.5 grams to 0.8 grams of protein per pound of their current weight per day, and depending upon the individual’s unique or specific medical and psychological condition. But remember, using GLP-1 medications is only the beginning, changing your eating behavior is key for achieving a stable, lasting and healthy weight.

Can You Change Your Metabolic Rate?

Many overweight individuals ask: are there foods that can change your metabolic rate? The answer is NO. There are no foods that can change your genetically set metabolic rate. Some agents like caffeine or stimulant medications can only briefly, very temporarily and slightly increase your current metabolism, but then after stopping them, the metabolic rate returns to your normal rate. Marketing and advertising promote agents that can burn fat quickly and without effort, but these agents will only waste your time and money and not result in any significant weight loss. The one exception is the dramatic weight loss results produced by the GLP-1 medications. Obesity & Cancer

While obesity is associated with many chronic metabolic diseases, it is also linked to the development and progression of numerous cancers. Overweight and obesity are associated with higher rates of cancer and account for 10% of new cancers each year in the United States. Obesity is linked with 12 types of cancers; colorectal, endometrial, postmenopausal breast, gallbladder, kidney, liver, esophagus, ovarian, pancreas, multiple myeloma and thyroid cancers. When fat cells become too enlarged and numerous in the overweight condition, they can over time, become stressed and dysfunctional and in a adverse microenvironment, such as poor oxygenation, etc., become damaged and release a host of pro-inflammatory hormones and proteins. They also release metabolic energy as fuel to developing cancer cells and can stimulate cancer growth and development. These inflammatory agents, known as cytokines, also decrease the ability of the body’s immune systems to detect and destroy abnormal and cancerous cells. These increased risks for cancer cell formation and growth can be reduced by achieving a decreased amount of fat cells through changing lifestyle behavioral eating patterns, medications, such as the GLP-1s, and /or bariatric surgery.

Do I have too much Cortosol?

Some of my patients believe that their weight gain and/or slow weight loss is caused by an excess amount of the adrenal hormone cortisol. But for the great majority of patients, this is not the case. Cortisol is a hormone secreted by the adrenal glands, and is increased in the rare case of the adrenal disease known as Cushing’s Syndrome or Cushing’s Disease, and to a lesser degree by increased levels of chronic stress. Cortisol hormone, known as the stress hormone, helps the body to deal with real physical trauma and damage, like accidents and physical injuries and also with psychological stresses like anxiety, worry, frustrations, depression and etc..Cortisol secretion and increased levels in these situations is a normal response. But prolonged elevated cortisol levels cause damage to the body. Some such effects are elevations of blood sugar, hypertension, skin changes and fat depositions primarily in the face, central body (with thin legs and thin arms), and a build up of fat at the back of the neck, known as a ‘buffalo hump’. But if you think that your weight gain could be caused by an elevated level of cortisol, although it is a very rare condition, ask your physician to check for a cortisol level in the blood or a 24 hour urine collection for ‘free cortisol’. The signs of cortisol excess are very visible and pronounced and the excess can be evaluated by lab testing. Many individuals gain excess weight by increased stress induced emotional eating and very few have their excess weight gain caused by excess cortisol levels.

Is it my THYROID

Many of my patients ask, ‘Is my Inability to lose weight caused by a low metabolic rate and a dysfunctional thyroid metabolism’? No, this is not the case. One very common metabolism myth is that a weight gain is caused by a dysfunctional and reduced metabolism. One’s metabolic rate is fixed and genetically determined at birth. The metabolic rate does not change and a suspected dysfunctionally low metabolic rate, such as with a low thyroid hormone level, is not a cause for weight gain and an inability to lose weight. To prove this reality, have your thyroid metabolism checked for a thyroxine (T4) and a thyroid stimulating hormone (TSH) level and be aware that for the great majority of individuals, perhaps more than 90%, the results will be normal. A higher calorie intake rather than a lower metabolic rate is the cause for a slower and reduced weight loss. Thyroid medication and other medications promoted to increase thyroid metabolism are not needed for weight loss.
