You don’t have to have all the answers. That’s one of the reasons your hospice team is there. 👏

Your nurse can help with changing symptoms and medications. Your CNA can assist with personal care. Your social worker can help with emotional and practical needs. Your chaplain can provide spiritual support. 🤍

Before we ever step into a patient’s home, our interdisciplinary team meets to review each patient’s needs, discuss changes, and create a plan of care together. By preparing ahead of time, we’re able to anticipate needs instead of simply responding to them.

They are often the ones spending the most time with patients, offering not only physical care but comfort, encouragement, and a familiar presence. ✨
