Peripheral nerves can regenerate, but that repair depends on Schwann cell support, a viable path for the regrowing axon and adequate blood supply. Gabapentin and pregabalin reduce pain for a subset of patients; they are symptom-directed and do not repair the nerve, so a diagnosis plus a pill is not the whole plan. Four things support the repair side: a blood supply check, because glycation (sticky blood) restricts blood flow to the feet; a B12 check, because long-term metformin use is associated with vitamin B12 deficiency and that deficiency may cause or worsen neuropathy; cell energy, because nerve cells are metabolically demanding and depend on mitochondrial energy production; and a repeatable baseline test, because measuring change requires repeating the same instrument the same way.
