One of the biggest reasons women avoid vaginal estrogen is fear.

Many have been incorrectly told that any estrogen requires progesterone, increases cancer risk, or is unsafe after menopause.

But low-dose vaginal estradiol is very different from systemic hormone replacement therapy.

Vaginal estradiol is designed to work locally in the vaginal and bladder tissues, where we have a very high concentration of estrogen receptors. These tissues absorb and utilize the medication directly.

With true low-dose vaginal estrogen therapy, systemic absorption is extremely low. In many women, blood estradiol levels remain unchanged or barely detectable.

Because of that, women using only low-dose vaginal estradiol generally do not need progesterone alongside it.

This is important because vaginal estrogen can dramatically improve:

🙏🏼Progesterone is one of the most misunderstood hormones in medicine. 🙏🏼

Many women are told that if they’ve had a hysterectomy, they no longer need progesterone. And technically, if you no longer have a uterus, progesterone is not mandatory for protection against endometrial cancer the way it is when estrogen is prescribed to women with an intact uterus.

But progesterone was never 👉🏼only👈🏼 about the uterus.

Progesterone has important effects throughout the entire body.

🧠 In the brain, it helps support calming neurotransmitters involved in sleep, anxiety, and irritability. Many women notice they sleep more deeply and feel more emotionally stable when progesterone is optimized appropriately.

Progesterone also plays a role in breast health, vascular health, and healthy cellular signaling throughout the body.

One of the biggest mistakes in hormone medicine is reducing hormones down to only reproductive organs. Hormones are systemic signaling molecules. They affect the brain, blood vessels, metabolism, connective tissue, sleep, mood, cognition, and long-term health.

That does not mean every woman with a hysterectomy automatically needs progesterone. But in clinical practice, many women feel significantly better when it is thoughtfully incorporated into their hormone replacement plan.

Low-dose vaginal estrogen is one of the most effective treatments we have for:

And unlike systemic estrogen, absorption into the bloodstream is extremely low.

The group where clinicians have traditionally been the most cautious is women with a history of endometrial cancer, since this cancer can be estrogen-sensitive.

That’s why this recent study is important.
Researchers followed younger women treated for endometrial cancer for more than 4 years and found no increase in recurrence rates in those using vaginal estrogen compared to those who did not.

This does not prove vaginal estrogen is universally safe after endometrial cancer, but it is reassuring data and supports a more individualized, evidence-based conversation for women struggling with severe vaginal symptoms after treatment.
