Manning Pharmacy’s Remote Patient Monitoring program is designed to help eligible patients better manage chronic conditions like high blood pressure or diabetes — while keeping their primary care provider involved.

This is not a replacement for your doctor.

Instead, our pharmacy team works alongside your P*P by helping monitor readings, following up with patients when needed, and sharing important updates so your provider has a clearer picture of what’s happening between visits.

Through this program, eligible Medicare/Medicaid patients may receive:

✅ Enrollment & training
✅ Easy-to-use cellular monitoring devices at no cost
✅ Daily review of blood pressure or glucose readings
✅ Follow-up from our pharmacy team when concerns come up
✅ Monthly summaries shared with your P*P

Our goal is simple: help patients stay supported, help providers stay informed, and help improve care between appointments.
