“Feeling better” doesn’t mean the same thing to everyone.
For one woman, it means finally sleeping through the night.
For another, it’s having enough energy to exercise again.
It might mean feeling stronger, enjoying s*x again, feeling more comfortable in your body, or simply recognizing yourself again.
That’s why good midlife care shouldn’t begin with a predetermined solution.
It should begin with understanding what has changed and what matters most to you now.

Why did I choose to build a direct-pay practice?

One of the biggest reasons was time.

Many of the concerns women bring to me don’t fit neatly into a short visit. There may be multiple symptoms, years of history, previous testing, different treatment options, and questions that deserve thoughtful answers.

I wanted to build a practice where I could spend that time with my patients.

At Whole Women’s Care, initial consultations are 60 minutes and follow-up visits are 30 minutes. We intentionally keep the practice small so that the care we provide can remain personal and accessible.

Direct-pay isn’t the right model for everyone. But it allows me to practice medicine in the way I envisioned when I created Whole Women’s Care: with more time to listen, evaluate, explain, and make decisions together.

A reminder that Whole Women’s Care is also a gynecology practice.

While much of my work focuses on perimenopause, menopause, hormonal health, and metabolic health, I’m first and foremost an OB/GYN.

That means we also care for many of the gynecologic concerns that don’t disappear just because you’ve entered midlife. This includes abnormal or changing bleeding, contraception, vulvar and vaginal concerns, s*xual health, cervical cancer screening, and preventive gynecologic care.

Sometimes those concerns overlap with hormonal changes. Sometimes they don’t.

Either way, women deserve a place where their gynecologic care can continue as their needs change.

Lab work can be incredibly helpful, but it is only one part of a good evaluation.

When someone comes in with changes in energy, sleep, weight, menstrual cycles, mood, or other symptoms, I don’t look at lab values alone.

We look at the full picture: your medical history, medications, menstrual patterns, sleep, nutrition, activity, metabolic health, and how things have changed over time. Laboratory testing, when appropriate, is only part of that evaluation.

Sometimes testing identifies something we need to address. Sometimes it helps us rule things out. And sometimes the most important information comes from your history and what has changed over time.

Good medical care is about putting all of those pieces together.

What does an initial consultation at Whole Women’s Care look like?

Our first visit is a full 60 minutes.

We start with an extensive evaluation of your history and what has been changing for you.

From there, we talk through what may be contributing to your symptoms, what deserves further evaluation, and what options make sense for you.

That may include reviewing previous labs or imaging, ordering additional testing when appropriate, and discussing treatment options, including hormonal and non-hormonal approaches.

You don’t need to arrive knowing exactly what you need or which treatment you want.

That’s what the visit is for.

“I just don’t feel like myself.”

This is one of the most common things I hear from women in midlife

That can mean very different things.

Some of those changes may be related to the menopause transition. Others may have another explanation entirely.

Good midlife care means looking at the whole picture, not attributing every symptom to hormones or simply accepting changes as part of aging.

It starts with listening, evaluating the whole picture, and figuring out what deserves attention.

That’s the approach we take at Whole Women’s Care.

A little look inside Whole Women’s Care.

When I designed this practice, I wanted the space to reflect the kind of care I hoped to provide here: calm, personal, comfortable, and unrushed.

We are a small, independent women’s health practice in Marietta, with a focus on perimenopause and menopause, hormonal health, metabolic health, and gynecologic care.

We intentionally keep the practice small so that each appointment allows time for real conversation, not just working through a checklist.

If you have recently found us, we are glad you're here.

If you’re new here, welcome to Whole Women’s Care.

I’m Dr. Maria Casiano, a board-certified OB/GYN and Menopause Society Certified Practitioner.

I created Whole Women’s Care to provide women with something that can be difficult to find: time to talk through what is changing, a thoughtful medical evaluation, and a plan that looks at the whole picture.

My practice focuses on women navigating perimenopause, menopause, hormonal changes, metabolic health, and preventive gynecologic care.

Visits are intentionally longer, and care is individualized.

The goal is good medicine, with enough time to do it well.

Whole Women’s Care is located in Marietta, Georgia, and we are currently welcoming new patients.

PCOS has officially been renamed:
Polyendocrine Metabolic Ovarian Syndrome (PMOS).

And honestly, the change reflects something many women have felt for years:
this condition was never just about the ovaries.

It can affect metabolism, insulin signaling, inflammation, ovulation, energy, fertility, and long-term health.

A whole-body condition was being described with a name that felt too narrow.

Names alone don’t change care.
But better understanding can.

Women’s health deserves language — and treatment — that reflects the full picture.
