Stability Health Engagement is key to managing the care of patients with chronic conditions, like diabetes, heart disease or obesity.

Helping clinicians and patients improve the effectiveness of cardiometabolic care, starting with diabetes, by using an innovative technology and coaching platform that significantly enhances patient engagement. It is for this reason that Stability Health has developed a successful care model that combines innovative technology with a coaching platform. Its technology platform contains a registry driven by a rules engine. The rules engine has embedded evidence-based standards of care from a variety of professional organizations, such as American Diabetes Association, American Heart Association, Association of Certified Diabetes Care and Education Specialists, and is able to ingest data from a variety of devices. Patients are then monitored when they are in their usual routines. The platform integrates Information collected through detailed patient assessment, driven by the rules engine, to create a comprehensive care plan. This plan supports patients in making lifestyle changes, and delivers specialist expertise to their clinical care teams, so that they make optimal, clinical decisions for medications and devices. In addition to the technology, Stability Health’s transformative care model supports patients with a human coach they connect with virtually using their preferred technology - phone, e-mail, text, video chat. Patients also have access to specialists to support more intensive education when needed, or requested - occurring at a time that is convenient for them. Medical specialists oversee and ensure care plans, and recommendations are appropriate for each patient. In this way, the entire care team, supported by the platform that standardizes ideal care, provides individualized care to each patient.

Check out our latest case study about the importance of .

Most patients don't fail to improve because they lack information.

They fail because change is hard — and no one meets them where they are.

Ms. D is 72, has , and has had every complication you can imagine. When we first started working with her, she was clear: "I eat what I want to and I don't like to exercise."

Her Stability Health coach didn't push. She focused on keeping the door open.

Gradually, Ms. D became more receptive. Small changes in her diet led to better glucose control she could see. That built her confidence to try something else. Six months later, her A1C dropped from 8.7% to 7.2%.

That's not a medication change. That's engagement — meeting a patient at their stage of readiness and walking alongside them until the work becomes theirs.

It's what we’re built for at Stability Health. Because the research is clear: outcomes don't improve until patients are active participants in their own care. Getting them there requires more than reminders and portals. It requires human connection, behavioral science, and time.

That's the model of success.

18 clinicians. Multiple practices. Three Days.

That's what a client visit looks like at Stability Health.

We don't wait for referrals to materialize. We go to where primary care teams are — in their buildings, between their patients — to understand what's working, what's not, and where we can remove friction.

Yesterday, that meant case discussions with new physicians still learning our program, a conversation with a newly minted practice manager before she's even started her role, and securing time with clinicians who were too busy to meet in person. (We'll be back for those.)

Primary care is overwhelmed. Getting physicians to meaningfully adopt the latest evidence for care requires more than awareness — it requires presence, repetition, and genuine partnership.

That's the work. And it's why our engagement model produces results that passive programs don't.

A value-based primary care organization serving a high-risk Medicare Advantage population had done everything right. Smaller patient panels. Enhanced access. Coding initiatives.

And yet — their most complex patients with and cardiometabolic comorbidities were running at 125% MLR, driven by excess hospital utilization. Their frontline clinicians felt stretched and underprepared for the complexity.

So they looked for a different approach.

Within 18 months of partnering with Stability Health, patients enrolled in the program for six months or more had 59% fewer hospital admissions compared to similar patients who never enrolled. For every two patients enrolled for six months, one admission was avoided annually — $14,000 in savings per avoided admission.

The lesson: even well-run, value-oriented practices have a ceiling on what they can achieve within four walls. Closing the gap requires extending care to where patients actually live — between visits, between appointments, between the moments when clinicians can reach them.

CKM (Cardiac-Kidney-Metabolic): Is not just clinical framing—it’s a wake-up call.

CKM connects what we’ve historically treated in silos:
➡️ Cardiovascular disease
➡️ Kidney disease
➡️ Metabolic conditions like diabetes

But patients don’t experience these as separate problems—and outcomes reflect that fragmentation.

CKM is:
• Progressive, often starting years before symptoms
• Driven by interconnected physiology—not isolated conditions
• Strongly influenced by social drivers of health
• Responsible for a disproportionate share of morbidity, mortality, and cost

Yet most care models still aren’t built to address it holistically.

At Stability Health, we partner with primary care organizations to operationalize CKM care, starting with diabetes by:
• Embedding evidence-based, longitudinal management
• Extending care teams with virtual-first support
• Closing gaps in access, adherence, and coordination

CKM isn’t a niche concept. It’s the dominant clinical reality.

Are your care models keeping up?

This National Doctors’ Day, we recognize and thank the primary care physicians who are at the center of everything we do at Stability Health. We can’t do what we do without your collaboration.

Primary care is where the real work of healthcare happens—managing complexity, coordinating care, and building long-term relationships that improve outcomes for patients living with cardiovascular, kidney, and metabolic (CKM) conditions. The physicians we partner with continue to demonstrate what’s possible when primary care is supported with the right clinical resources, data, and care models.

We’re grateful for the trust our physician partners place in us and proud of the outcomes we’ve been able to achieve together. Their willingness to collaborate and rethink traditional care delivery is helping set a new standard management of patients with CKM.

If you are exploring new ways to improve outcomes for patients with complex cardiometabolic needs, and reduce burden on frontline PCPs, we’d welcome the opportunity to connect.

Today is Diabetes Alert Day—a reminder not just of how common diabetes is, but of how poorly we are managing it as a healthcare system.

Despite being one of the highest-spending countries in the world on , the U.S. has experienced rising rates of preventable complications from over the last decade. For millions of patients, access to care does not consistently translate into better health.

This is not a knowledge problem. We know how to manage diabetes. It is a care delivery problem—fragmented support, limited time in primary care, and too little help for patients between visits when most self-management decisions actually happen.

At Stability Health, we’re focused on closing that gap. By partnering with primary care practices and providing continuous, virtual-first support for patients with cardiometabolic conditions, we help translate evidence-based care into day-to-day reality. The result is not just better glycemic control, but fewer hospital admissions and emergency department visits.

Diabetes Alert Day should be a call to action for all of us in healthcare: better outcomes are possible, but only if we change how care is delivered.

Stability Health partners with primary care practices to improve outcomes for patients with Cardiovascular-Kidney-Metabolic (CKM) conditions.

Within six months, patients had a 59% reduction in hospital admissions and a 30% reduction in ED visits. We also drive meaningful improvements in glycemic control, while extending the care team with virtual-first support that helps stay more connected to their patients.

She brings extensive knowledge and experience driving meaningful improvements in and financial performance in value-based primary care groups. As Chief Nursing Officer at IMA Medical Group (a full-risk Medicare Advantage primary care group serving a predominantly Hispanic population in central Florida) she implemented the Stability Health program, which led to high seven-figure savings from lower hospital admissions, and less frequent emergency department utilization.

“Sharon will work with prospective clients to identify how they will save money and benefit by partnering with us,” said Peggy Chou MD, President and Chief Medical Officer.

“After a year sabbatical in Thailand, I’m pleased to be back in the world of value-based care,” said Sharon. “I’m particularly excited about joining Stability Health. Stability Health is unique in its approach to diabetes, which is the root cause of many cardiometabolic conditions. It was the only program we found that addressed the needs of patients, primary care , and the physician organizations responsible for outcomes.”

Previously, Sharon was VP, Chief Clinical Officer at Wellcare Medicare Health Plans responsible for care management throughout the organization. She also led the analytics and informatics teams and built WellCare’s platform, which led to improved accuracy and efficiency of the utilization management process.
