For many patients in our area, access to a primary care physician remains a challenge. Without continuity of care, managing chronic conditions and coordinating healthcare can become overwhelming for both patients and families.
Senior Care Partners offers primary care services for patients over 65 who do not currently have a primary care provider. Our approach is individualized, accessible, and focused on maximizing function, independence, and quality of life.
Patients currently being cared for by our providers in the hospital or skilled nursing facility may arrange a new patient appointment at discharge. Patients new to our practice are encouraged to call our clinic to schedule an appointment.
