As part of the CareOregon family, JCC empowers people and systems to improve the health of our community. In Jackson County, OR, CareOregon and Jackson County Mental Health joined together to serve Oregon Health Plan members as Jackson Care Connect. Jackson Care Connect works with the state of Oregon to provide health services to members of Oregon Health Plan (OHP), the state's Medicaid plan. With
the help of members, providers, hospitals and communities, Oregon has created a new type of health plan called a Coordinated Care Organization (CCO). Jackson Care Connect is a CCO, where you can get all of your health care from the same plan, resulting in better, more effective care. This means that you receive physical, mental and dental health services from Jackson Care Connect. Provider groups, hospitals, health plans, mental health organizations and agencies in Jackson County have joined together to serve you through Jackson Care Connect.
