Jackson Care Connect Jackson Care Connect (JCC) provides physical, behavioral and dental health care for Oregon Health Plan (Medicaid) members in Jackson County, Oregon. to 5 p.m.

As part of the CareOregon family, JCC empowers people and systems to improve the health of our community. In Jackson County, OR, CareOregon and Jackson County Mental Health joined together to serve Oregon Health Plan members as Jackson Care Connect. Jackson Care Connect works with the state of Oregon to provide health services to members of Oregon Health Plan (OHP), the state's Medicaid plan. With the help of members, providers, hospitals and communities, Oregon has created a new type of health plan called a Coordinated Care Organization (CCO). Jackson Care Connect is a CCO, where you can get all of your health care from the same plan, resulting in better, more effective care. This means that you receive physical, mental and dental health services from Jackson Care Connect. Provider groups, hospitals, health plans, mental health organizations and agencies in Jackson County have joined together to serve you through Jackson Care Connect. To ensure you get the best care, we’ll coordinate your care with all of the providers or facilities you use. More nutrition support is now available for eligible Oregon Health Plan (OHP) members.

Through the Pantry Stocking Benefit and Fruit and Vegetable Benefit, eligible members may receive support like fresh fruits and vegetables, pantry staples, grocery boxes or prepaid cards for up to six months.

We're proud to partner with local organizations to help deliver Health-Related Social Needs (HRSN) nutrition benefits to eligible Oregon Health Plan (OHP) members.

What if mental health support could start in your primary care clinic?

That’s the idea behind PHLOW (Psychiatric Health, Life Skills and Opportunities for Wellness), an innovative model developed by La Clinica in partnership with Jackson Care Connect. By embedding psychiatric consultation into primary care, PHLOW is helping improve access to mental health care, strengthening provider confidence and improving outcomes for patients.

In 2025, 531 patients were served under the PHLOW model and participants showed drops in anxiety and depression, along with a reduction in mental health claims.

Children and teens ages 3 to 19 should see their primary care provider for one well-visit each year. These preventive checkups track physical growth, emotional and mental development and overall health. They help providers find health concerns early and ensure your child is up to date with their immunizations.

Safe, stable recovery starts with having the right support at the right time.

Through a partnership with Rogue Retreat, we are proud to help provide medical respite support for members experiencing houselessness who need ongoing care and recovery support outside of a hospital setting.

In 2025, the program served 46 members and helped connect individuals to more stable housing, substance use treatment and ongoing support services.
