The hospital stay ends. Who looks after your recovery once you’re home?

You leave the hospital with discharge instructions and a follow-up appointment that may be weeks away. What happens in between?

After nearly 28 years of living with cancer, Jane Loeb Rubin knows what it feels like to leave the hospital. Until recently, going home meant navigating much of her recovery on her own. A former hospital executive who knows the healthcare system from the inside - she has called the return home a “free fall.”

Jane is also the mother of our CEO and co-founder, Caroline Taylor Hodge.

This summer, after another hospital stay, she had a clinical team supporting her recovery at home.

Three years ago, we saw our first patient. That one visit set everything in motion.

🚨 Air Quality Alert: The AQI in our area has climbed into the "unhealthy" range today (160+) as smoke from Canadian wildfires moves through NJ/NY.

If you're recovering from a recent hospital stay or managing a chronic condition — heart disease, COPD, asthma — poor air quality can hit you harder than most. Your lungs and heart are already working overtime; don't make them work through smoke too.

Our advice: stay inside if you can. Dimer Health is here virtually — no need to risk exposure for a check-in, medication question, or symptom concern. We can support most of your medical needs from the comfort (and clean air) of your own home.

If you do need to step out, keep these in mind:
✔️ Limit your time outdoors
✔️ Skip strenuous activity — save the walk or workout for a clearer day
✔️ In the car, switch your air to "recirculate/internal" so outside smoke doesn't get pulled in
✔️ Stay hydrated — it helps your body manage the extra strain
✔️ Keep windows and doors closed at home, and run an air purifier or AC on recirculate if you have one
✔️ If you're at higher risk (recent surgery, heart or lung condition) and must go out, an N95 mask offers real protection — a cloth mask won't cut it for smoke
✔️ Know your warning signs: increased shortness of breath, chest tightness, dizziness, or a worsening cough means it's time to call us — don't wait it out
