Pt presents on Wednesday after-hours
33 yo male
MHx: NSF
DentalHx: Hasn’t been to dentist for over 10 years
CC: Extreme pain in LL. “I was at the ER yesterday and they put me on an antibiotic, but I feel like the swelling is going under my chin”
Extraoral: Swelling in LL cheek/lip; no difficulty breathing noted

Intraoral/radiographs:
Tooth 19 previous RCT with PARL; no pain to percussion
Tooth 20 PARL and pain to percussion/palpation. Large swelling centered on apex of tooth.

Diagnosis:
Tooth 19 asymptomatic failed root canal; questionable prognosis for retreatment and referral to endodontist
Tooth 20 large DOBL caries to bone level, Necrotic with Symptomatic Apical Abscess; poor prognosis for RCT, BU, crown lengthening, crown

Pt chose to incise and drain abscess and extract teeth 19 and 20. See video for start of incise and drain!

April 4th 2026 marks the GRAND OPENING of Idaho Emergency Dental. This team made it possible! They were resilient and compassionate throughout a day of new trials and tribulations. What a blessing!

We got some patients out of pain with extractions, recemented some crowns, made a patient a new temporary crown, and tested our office systems!
