Today, Amanda and I had the opportunity to provide education and clinical insight to the management team at regarding the distinct roles of Case Management versus Care Management, two disciplines that are frequently misunderstood within the healthcare continuum.

Case Managers are most commonly found within hospitals, skilled nursing facilities, rehabilitation settings, and insurance organizations, where their primary focus is discharge planning, utilization management, resource coordination, and ensuring appropriate transitions of care.

Care Management, by contrast, is primarily a post-discharge service delivered within the home setting, focused on ensuring continuity, safety, and long-term success once a client returns home. While often associated with transitional support and long-term care coordination, Care Management extends far beyond basic follow-up. It is the comprehensive coordination, oversight, and ongoing supervision of healthcare services to ensure every aspect of a client’s care functions cohesively and effectively within their home environment.

This process includes close collaboration with physicians, providers, families, caregivers, and community resources to promote patient safety, continuity of care, adherence to treatment plans, and successful implementation of recommended services.

Our mission is rooted in education, advocacy, and client-centered support. By helping healthcare professionals, families, and organizations better understand the value of Care Management, we strengthen outcomes and improve the overall care experience for the clients we serve.

Healthcare is constantly evolving, and one of the greatest gaps we continue to see in our communities is the need for consistent oversight, advocacy, and support for aging adults who may not require full-time care , but still need someone keeping a close eye on their well-being.

For months, our team has worked around the clock to develop programs that truly meet the needs of families, clients, and the community as a whole. Our mission has always been centered on preserving independence while ensuring safety, dignity, and appropriate oversight of care.

We are proud to announce the launch of our Care Manager On-Call Program.

This program was specifically designed for individuals who may not yet need extensive daily assistance, but whose families want peace of mind knowing there are trusted professionals available to provide “boots on the ground,” wellness oversight, emergency support, and care coordination when loved ones live out of town or simply have demanding schedules.

🚨 Our Care Manager On-Call Program will provide:

This week, we want to give a huge and heartfelt shoutout to our incredible nurse, Amanda Butas, CDP, GCM, ALM, BCPA.

Over 12 years ago, I had the privilege of meeting Amanda, and ever since, we’ve been inseparable. From working side-by-side, referring clients to her GH when I worked in a SNF, helping her run her Home Care business, stepping in at her Group Home while she traveled the world, going on business work trips together, and eventually starting Kaizen Care Management, it has been an unforgettable journey filled with purpose, growth, and endless laughs.

Amanda’s knowledge, compassion, and dedication are truly unmatched. Behind the scenes, she works tirelessly to ensure the safety, care, and well-being of every single client we serve. Her heart for people and commitment to excellence make an impact far beyond what most ever see.

We truly couldn’t be luckier to have someone so passionate and devoted serving our clients and community.
