One of my professional philosophies is simple:

Comprehensive chart audits provide the visibility needed to uncover trends, gaps, and opportunities that might otherwise go unnoticed.

A documentation issue may start in a chart, but the root cause is often found in the process behind it.

That is why I perform comprehensive chart audits and help agencies develop meaningful Performance Improvement Projects based on what the data is actually showing.

The goal is simple: identify opportunities early, strengthen systems, and stay ahead of problems instead of reacting to them.

A home health agency's Quality Assurance and Performance Improvement (QAPI) process is as important as its accounting process. Just as your books are audited and reconciled for compliance, informed forecasting, and risk mitigation—the same concept must apply to your QAPI program.

Remember when CMS added QAPI to the Conditions of Participation in 2017, making a data-driven program mandatory by 2018? The agencies that truly embraced it operationally are much better positioned in 2026 with the recent CMS changes and increased scrutiny across the industry.

Often QAPI becomes a binder collecting dust when you are in the weeds with the day-to-day. But this is not your fault — it likely was not taught the right way, and some agencies have normalized placing this program on a shelf, only to revisit it every two to three years. Had you been taught this as a foundational process and not a peripheral task, it would be as automatic as the books.

An effective QAPI program is the backbone of healthcare operations. It allows agencies to step back and clearly see what is working, what is not, and where problems are starting to develop. This ensures consistent delivery of high-quality care, supports proper reimbursement, and helps agencies build the kind of structure that attracts and retains highly talented staff.
