"But I’ve started to think the most important question on the intake form is the one that isn’t printed there. Not list your previous surgeries, with its three tidy lines, but something closer to: What has your body been through? — followed by a blank page." - Dr. Tiffany Mullen, Beneath the Surface

Some stories don't fit into a checkbox.

In her latest Substack, Dr. Tiffany Mullen explores why the most important parts of a patient's health history are often the ones medicine leaves out—and why trauma, symptoms, and healing are more connected than we've been taught.

None of these biomarkers are exotic. In fact, most are widely available and often affordable.

So why aren't they included in a standard annual physical?

Because conventional care is designed to detect disease once it's already present—not to monitor the subtle shifts that happen years before symptoms appear.

When these markers are tracked together—and followed over time—they tell a much more complete story about your health.

Chronic low-grade inflammation is now understood as a common upstream driver of most age-related diseases. It is not the only cause of any of these conditions, but it is a shared root system across almost all of them.

The reason this matters, practically, is that if you can measure and lower chronic inflammation, you are addressing a shared driver of multiple age-related conditions at once. Which is exactly the kind of leverage proactive medicine is built around.

Genetic testing looks at small variations in your DNA that affect how efficiently certain enzymes, receptors, or pathways function in your body. In metabolic health specifically, we look at variants that influence how you process nutrients, how efficiently you use vitamins and cofactors, how your hormones are metabolized and cleared, and how susceptible you are to specific chronic conditions.

Are you interested in learning more about how genetic testing can help you understand your metabolic health? Read more: https://vytalhealth.com/metabolic-health-and-genetics-which-genes-actually-matter-and-what-to-do-with-what-you-learn/

Inflammation isn't always the enemy. In fact, acute inflammation is one of your body's most important healing tools.

The problem is when inflammation never fully turns off.

This is called inflammaging: a chronic, low-grade inflammatory state that quietly develops over time. Unlike the inflammation you feel after an injury or illness, inflammaging works in the background for years, often without any obvious symptoms.

You can't feel inflammatory markers slowly rising. You can't feel the gradual damage happening within your tissues. But over time, this persistent inflammation can contribute to many of the chronic conditions we associate with aging, including heart disease, type 2 diabetes, and autoimmune disorders.

The good news? Chronic inflammation isn't simply an inevitable part of getting older. Understanding what's driving it is the first step toward changing its trajectory.

There are millions of variants in the human genome. Most do not have meaningful clinical relevance for day-to-day metabolic health. The handful that do are worth knowing about because they change how a physician builds your plan.

At Vytal Health, we can help you determine the correct testing to do, and how to translate those results to improve your metabolic health. Learn more, or schedule a discovery call: https://vytalhealth.com/metabolic-health-and-genetics-which-genes-actually-matter-and-what-to-do-with-what-you-learn/

There is real evidence that stress and trauma modulate the immune system and can raise the risk of autoimmune disease. We also discovered in 2024, that the molecular machinery women’s cells use to silence their second X chromosome — a complex built around a molecule called Xist (X-inactive specific transcript) — itself provokes the immune system, helping to explain the striking female bias in autoimmunity

But it gets even more nuanced than that.

Your immune system is attacking your own body.

Dr. Tiffany Mullen deconstructs that sentence in her latest substack article "Mistaken Identity". Because she's come to believe it is both true and profoundly misleading, and that the space between the two is where a great deal of unnecessary suffering lives.

Two people can follow the exact same nutrition and exercise plan and get completely different results, and a lot of that difference is written in DNA. Specifically in the genes that shape how you process carbohydrates, how you metabolize fats, how efficiently you use B vitamins, how your hormones clear, and how your body responds to stress and inflammation.

The genes themselves are only half of the picture. What you do with the information is the other half. And with the rise of accessible genetic testing, more patients than ever are trying to make sense of raw data reports without a physician to help them interpret what actually matters.

What if your symptoms aren't random, but your body's way of asking for the all-clear?

Dr. Tiffany Mullen shares the story of a patient whose "normal" lab work couldn't explain what she was living through, and why healing begins when we learn to see the whole picture.
