What does “tongue overflow” tell us about tongue space and airway health?

This image highlights a screening tool we use within the FAirEST 6+4 to help evaluate tongue space during Lingual Palatal Suction (LPS). When the tongue extends beyond the teeth on both sides, it can be a strong indicator of insufficient tongue space and may suggest the need for further airway evaluation. Functional screening tools like this help clinicians better recognize patterns connected to sleep-disordered breathing, oral function, and overall development.

Congrats to our most recent Certificate Holder in Oncology Oral Care - Sara Williams, BSDH, RDH, CH-ONC from Mt. Vernon, WA! She is the owner of Cornerstone Myo and she holds certificates in The Cotten Method of oral cancer screening as well as this new credential.

Sleep and breathing are deeply connected.

The relationship works both ways: the shape and function of the airway can influence the quality of sleep, and sleep itself can influence airway health. When breathing is restricted during sleep—due to airway collapse, poor oral posture, or structural limitations—the body may struggle to reach deep, restorative sleep.

Over time, this cycle can contribute to symptoms such as:
• Snoring
• Mouth breathing
• Restless sleep
• Daytime fatigue
• Jaw tension or facial development concerns

That’s why evaluating the airway is such an important part of understanding sleep-related symptoms. Identifying underlying airway factors allows for more targeted care aimed at improving both breathing and sleep quality.

Congratulations to Soroush Zaghi, MD and The Breathe Institute team on publishing their 2025 update on lingual frenuloplasty (tongue-tie release with sutures) outcomes.

They originally reported a retrospective analysis of 445 patients treated in conjunction with myofunctional therapy in 2019 - a groundbreaking foundation for the field.

This new prospective study of 445 patients aged 4 years and older builds on that work with several key refinements:

✅ Standardized pre- and postoperative myofunctional therapy protocol with readiness assessments to ensure consistent preparation and rehabilitation of tongue and orofacial muscles.
✅ Functional mobility endpoints - the Tongue Range of Motion Ratio (TRMR) and Floor of Mouth (FOM) Hold Maneuver - used to improve diagnosis, guide surgical planning, and determine adequacy of release.
✅ Preserving more fascia and a transition from scissors only to assistance with a LightScalpel CO₂ laser for greater surgical precision, visualization, bleeding control, and less complications.
✅ Use of a tissue glue and reduced sutures for enhanced wound stability, bleeding control, and improved wound healing.

The field continues to progress through the pioneering work of this team, collaboration with myofunctional therapy, and ongoing innovation in assessment and technique.

These releases are a very gratifying part of my work, and it is very inspiring to see how we can keep learning and evolving to achieve even better outcomes for our patients.

When the airway is compromised, the effects don’t always show up where you expect.

Sleep, behavior, focus, mood, growth, and overall health are deeply connected to how we breathe—especially during sleep. What may look like isolated symptoms are often branches of the same root: a compromised airway.
