4 years…!!! 🤩🤩🤩

05/31/2026

The Office is turning 4 years old on June 1 and I wanted to celebrate it by doing a short series that is 4 parts to show the Office around.

Today we are starting with the outside, reception, the waiting room and the x-ray area.

Please let me know if you have any questions about anything you see.

What do you think about the office? Anybody wants to know what it looked like before?

The Best Tape for Plantar Fasciitis 🦶✨

Step 1: Set the Position
-Put the foot in a straight position with the toes pointing up toward the nose. Maintain this position throughout to properly build and support the arch.

Step 2: Apply the Vertical Anchors
-Using 2-inch athletic tape, start at the front of the foot (near the ball) and smooth it straight down to the base of the heel. Repeat this to place 3 bands across the bottom of the foot.

Step 3: Create the Support Ribbons
-Start a new strip at the front-middle of the foot. Run the tape down toward the arch, wrap it securely around the back of the heel, and bring it back up to the front where you started. Repeat this ribbon pattern 3 times to maximize arch support.

Step 4: Secure with Transverse Strips
-Starting from the base of the heel and working your way up, apply small, overlapping transverse (horizontal) strips across the sole to lock everything in place.

Step 5: Lock the Edges
-Apply a final anchoring strip around the top edge of the tape design to ensure nothing moves and everything stays completely secure for a few days.

A fungus party? 🤔🎉 Not exactly the kind you want an invite to.

~Comment PARTY below and we’ll DM you the link to our full video breaking down the 8 Warning Signs of Toenail Fungus you need to look out for! 👇

If you’ve noticed a dark, thick, or powdery spot on your toenail that has a distinct odor, you might be looking at a Mycetoma.

In the podiatry world, a mycetoma is essentially a massive, highly concentrated buildup of fungal organisms locked underneath the nail plate. Here is what is actually happening:

• The Discolored Powder: That crumbly, yellowish-brown or orange debris under the nail is a direct buildup of active fungal elements and skin debris.

• The Biofilm Shield: This high concentration of fungus creates a "biofilm"—a dense, highly protective matrix that acts like a shield. Because of this structural shield, standard treatments have a very hard time penetrating, which is why it requires targeted topical treatments to break through.

• The Malodor: Yes, that distinct smell is a byproduct of the massive concentration of fungus thriving in that dark, enclosed environment.

A mycetoma is a sign of a more advanced infection, but it's just one of many ways fungus shows up.

Ankle sprain? What you do AFTER matters just as much 👇

✔️ Elevate above your hip when sitting
✔️ Elevate above your heart when laying down
✔️ Ice 10–20 min per hour (NOT longer 🚫)

👉 Cast or dressing? Ice behind the knee instead — the cold won’t pe*****te through

Doctor Review: Are Brooks Ghost worth it?

Short answer: YES ✔️
Dr. Richard wore them herself while recovering from plantar fasciitis during the acute phase — and they passed the real-life test.

Why we recommend them:
• Excellent cushioning (especially at the heel)
• High heel-to-toe drop → ideal for heel strikers
• Neutral design → best for high or neutral arches
• Great option if you wear custom orthotics

Who they’re NOT best for:
Severe flat feet or excessive overpronation may do better in a stability shoe.

Pro tip: If you use orthotics for flat feet or overpronation, we often suggest a neutral shoe so the orthotic sits flat and works properly.

Best for: walking, running, plantar fasciitis, Achilles tendonitis, and general foot pain.

Verdict: Worth it ✔️

Bunion surgery has evolved a lot over the years. 👣

One technique that is becoming very popular is MIS bunionectomy (Minimally Invasive Surgery).

Instead of a large incision, the bone is cut through a very small incision using a specialized burr, and then the first metatarsal is realigned into a better position.

When you look at the X-ray after the correction, it can sometimes look like the first metatarsal head is “floating” in the space between the bones, which can look surprising at first.

But there’s an important structure that you can’t see on X-ray: the periosteum.

The periosteum is a soft tissue layer that surrounds the bone, and it remains intact during the procedure. Because it’s soft tissue, it doesn’t show up on X-rays, but it plays a very important role.

It helps provide stability to the osteotomy and maintains a great bl00d supply, which supports healing after surgery.

Modern bunion surgery is very different from what it used to be.

Find the Diagnosis - Case 8
-The Double Challenge 🕵️‍♀️🦶

We’re back with another round of “Find the Diagnosis!”

This case is a tricky one because it’s a total master of disguise. It often presents with symptoms that look like one thing, but the structural cause is actually something else entirely! 🦴

The Clues:
1️⃣ Pain at the ball of the 2nd toe
2️⃣ 2nd toe starts to lift off the ground
3️⃣ 2nd toe becomes hammered
4️⃣ It begins drifting toward the big toe
5️⃣ Occasional shooting pain or numbness on the 2nd / 3rd toes ⚡️

Wanna go the extra mile? 🧠
We want TWO answers in the comments:
1. What is the actual DIAGNOSIS?
2. What is this condition OFTEN MISDIAGNOSED as?

I know we have some fellow podiatrists and medical students in the community—let’s see if you can nail both! 👊

The official reveal is coming tomorrow. Drop your guesses!
