If others can hear your breathing, take note. It’s time to take action. 🩻

That is very suggestive of a narrowing in your airway, the pitch of the sound, whether it happens with an inhaler vs an exhale, can help me as a laryngologist localize the obstruction it to your lungs vs upper airway.

One condition I identify and treat as a laryngologist is called idiopathic subglottic stenosis. This is a chronic, inflammatory, progressive scarring condition of the upper trachea. It is most commonly presents in Caucasian females, otherwise healthy, ages 30-50. It’s often misdiagnosed as asthma or anxiety. Often women will present when symptoms have worsened leading to significant shortness of breath, noisy Darth Vader breathing (stridor), and difficulty with mucus trapping.

Their airway can often be described as ā€œbreathing through a strawā€ by the time a proper diagnosis is made. This can be an airway emergency. Medical gaslighting to the extreme. 😔

Currently the mainstay is surgical dilation with in office steroid injections showing promise for reducing inflammation. Most likely it’s etiology is immune dysregulation but research into this is ongoing. Unfortunately, rates are high for recurrence making this truly a chronic condition.

Can difficulty swallowing be ā€œsilent?ā€

Unfortunately, it can.

Oropharyngeal dysphagia refers to changes in the efficiency and safety of swallowing caused by throat muscle, throat sensation, or changes in both.

So difficulty swallowing due to the throat. Not the esophagus.

Changes can be subtle, ie cough when drinking liquids, longer to finish a meal than others, weight loss, some buildup of mucus after meals. Not the classic steak getting stuck sensation. So, it goes unchecked.

As a laryngologist (ENT who specializes in difficulty swallowing) I never simply ask, ā€œdo you have trouble swallowing?ā€ I ask about how eating and drinking feels, is it effortful, is there coughing, does it feel normal to the patient.

This type of difficulty swallowing can develop in unchecked reflux but can also be seen in neurological diseases (stroke, Parkinson’s Disease), and even with aging.

Uncontrolled, it can not only affect quality of life, but can lead to malnutrition, weight loss, even pneumonia.

We can identify this classically with a swallow study known as a modified barium swallow study or a camera assisted swallow study known as a FEES.

Early identification is key and interventions such as swallow therapy with SLP can be helpful.

Being an ENT isn’t easy, being an ENT patient is even harder!

Preemptively taking care and protecting systems, such as your voice, your hearing, your nasal breathing, is hard when these are things we take for granted.
But when nasal congestion leads to difficulty sleeping, hoarseness makes it hard to teach and do your job, and hearing loss makes it hard to engage with your grandchild, this is when we realize how important these systems are.

So what separates a ā€œgoodā€ patient from a ā€œbadā€ patient?

One who understands prevention is better than treatment, consistency is king, and seeking care is always the right answer.

Did you know your stomach and esophagus are angled in a way to protect you against reflux?

There are a few different natural mechanisms your body has to protect you against reflux but one of the main barriers to reflux is also the first failure: the gastro esophageal junction and the lower esophageal sphincter.

In normal anatomy, we see an acute angle between the esophagus and top of the stomach. This anatomy is crucial. After food passes into the stomach, this angle, allows the top of the stomach to ā€œballoon upā€ under your diaphragm, transmitting pressure to close the flap and seal the stomach.

Unfortunately, for a variety of reasons, including just living…the ligaments that hold this junction loosen over time changing this angle and shape, making it harder for the valve to close. Barrier failure. šŸ˜”

Gel alginates are a natural way to help this barrier. They won’t strengthen the ligaments, but they help to create a new barrier to what is in your stomach and prevent them from escaping out of the stomach. Alginate gels float to the top creating a nice little ā€œraft.ā€

To me she isn’t just a trailblazing woman in otolaryngology, nor a strong and determined woman who faced obstacles most of us can only imagine, but she’s the most loving ā€œride or dieā€ mom out there. Growing up, and even now in adulthood, I know that any problem, never too big nor small, mom won’t just have my back she will help me find a solution. And, I know that no matter what the world throws at me, I will be surrounded by her love and support.
