🔎 This patient came to us from New York after previous procedures with another surgeon focused heavily on her thighs.

The result was an imbalance in her proportions, areas of over- and undertreatment, and significant fibrosis and scar tissue.

For her revision, the goal isn’t simply to remove more fat. We’re addressing the scar tissue and fibrosis, carefully recontouring the thighs, restoring a more even layer beneath the skin, and using J Plasma for additional skin tightening when appropriate. The results even 3 days post-op are a significant improvement.

Nervous about surgery without general anesthesia? đź’¬

With our approach, patients receive carefully monitored sedation from an anesthetist or anesthesiologist rather than full general anesthesia.

Sedation is then adjusted to keep you comfortable and relaxed throughout the procedure, while allowing our surgical team to carefully position the patient.

Avoiding general anesthesia can also allow for more precise positioning and a cleaner surgical approach. General anesthesia is rarely needed and is used in only about 1% of cases.

The pelvic girdle plays a critical role in the presentation and treatment of lipedema.

When congestion and fibrosis build in this area, addressing it surgically can be an important part of restoring healthier tissue and improving lymphatic flow.

For this patient, we initially focused on the thighs because that area had significant congestion and fibrosis. The next step is to address the pelvic girdle.

By treating the pelvic girdle, we can work to decongest this important transition zone and continue improving the areas affected by lipedema.

💡 Which area should we treat first? There isn’t one answer for every patient.

When I develop a treatment plan for lipedema reduction surgery, I look closely at each patient’s pattern of fibrosis and congestion to determine the most appropriate sequence.

For some patients, I may begin with the calves, ankles, or arms, where fibrosis is more significant. For others, congestion is more prominent through the thighs, hips, or torso, making those areas the priority.

My goal is always to develop a treatment sequence that is specific to your anatomy and needs, not simply follow a standard order.

Where do you feel the most fibrosis? Let us know in the comments below. 👇

Calves and ankles are some of the most technically challenging areas to treat with liposuction and precision matters.

The tissue in this area is thin, with important nerves, tendons, blood vessels, and lymphatic structures nearby. That’s why staying in the correct “safe plane” is essential.

I perform this treatment using tumescent anesthesia with twilight sedation, allowing me to work carefully and precisely while avoiding the deeper structures.

We also use anticoagulation protocols to help reduce the risk of complications such as DVT, and encourage patients to begin walking shortly after surgery.

Treating the calves and ankles requires experience, precision, and a careful understanding of the anatomy.

🙌 I’m thrilled to announce that The Roxbury Institute will be in-network with effective October 15, 2026. Eligible patients seeking specialized lipedema treatment in California will have access to care with me, Dr. Aria Vazirnia, and The Roxbury Institute Surgical Center in Los Angeles. I’m grateful for the opportunity to help make specialized lipedema care more accessible to those who need it.

Lipedema doesn’t just affect the legs. It can also involve the pelvic girdle, an area that is often overlooked but plays a critical role in both function and appearance.

In this Stage 2 patient with a Type 3 lipedema pattern, the disease extends from the hips and pelvic girdle down into the legs. If pelvic girdle involvement is missed during surgery, it can leave behind untreated disease and cosmetic imbalances.

Every lipedema patient has a unique pattern of disease. That’s why a comprehensive evaluation and surgical plan are so important.

What an incredible honor to receive the Outstanding Achievement in Medicine Award at the Daytime Beauty Awards. 🏆

Thank you to the Daytime Beauty Awards for recognizing my work and for creating a platform that celebrates innovation and impact in medicine and beauty.

It was especially meaningful to have my dear longtime friend and former colleague, Dr. Paul Nassif, present the award. His heartfelt words made the evening unforgettable.

I welcomed the opportunity to use my acceptance speech to bring greater awareness to lipedema, a condition that still remains far too misunderstood, under-recognized, and misdiagnosed. Every opportunity to increase understanding matters, because patients deserve earlier answers, accurate diagnosis, and access to informed, compassionate care.

I’m deeply grateful to my patients, colleagues, and entire team who continue to make this work possible and help move the understanding and treatment of lipedema forward. 💜
