The stands are full again on Friday nights, and some of y'all are doing math before you ever leave the house.

Don't drink anything after 4p. Sit on the aisle. Count how far the restroom is from your seat. Leave at halftime because the line gets long, and you can't wait in a long line.

Nobody sees that math. You just look like somebody who likes the aisle seat.

Here is a cost most small business owners in South Georgia never see on a spreadsheet.

An employee wakes up feeling rough on a Tuesday. Nothing dramatic. A sinus thing, a rash, a knot in a shoulder that will not let up. They call around, get an opening for Thursday afternoon, if they're lucky, and that Thursday turns into a half day gone. Drive time, waiting room time, the pharmacy stop on the way back.

Multiply that across a crew of 25 over a year, and you are paying for a lot of hours that never touched the job.

We get asked two things all the time, so let's clear them up.

First: no, you do not have to be an Orchard Health member to see Rebecca. Aesthetics is open to the whole community. If you have never set foot in our office and you have no plans to change your doctor, you are still welcome to book. Our members are family, and we treat everybody who walks through that door as they could be.

Second: your first visit is a conversation, not a procedure.

Here is what actually happens. You sit down with Rebecca. She asks what you notice in the mirror and what you would want to be different, in your own words, not off a menu. She looks at how your face moves. She talks through what she would suggest, what she would not, and what a realistic result looks like for you specifically. She will tell you if she thinks something is not worth doing.

And then you leave. That is allowed. You can go home and think about it for three weeks. Nobody is going to call and bug you.

That consultation is complimentary, and there is no pressure attached to it. Aesthetics appointments are separate from primary care, so it does not touch your visit schedule or affect anything on the medical side.

The reason we do it this way is simple. The people happiest with their results are the ones who understood the plan before anyone picked anything up. What is right for you, and whether any treatment is right for you at all, gets decided in that one-on-one visit.

A member came in a few weeks back needing to see a specialist. Nothing scary, but not something to sit on either.

Here is the part that matters. She did not leave with a piece of paper and a phone number and a "good luck."

Angela got on the phone. She found out who was taking new patients, what they needed sent over, and when they could actually get her in. She faxed the records the same afternoon so nobody would call our member 2 weeks later asking for something we already had. And when the appointment was set, we let her know.

Then, and this is the part people are usually surprised by, we followed up afterward. Because a referral is not finished when the appointment is made. It is finished when we know what happened and what comes next.

That is not a special favor. That is just how we do it here. When you have a smaller panel, and you actually know the people in your chart, this kind of thing is possible. Nobody is falling through the cracks in a stack of 400 messages.

Getting to a specialist can be one of the more frustrating parts of healthcare. We would rather carry that weight for you than hand it to you on a sticky note.

Somewhere along the way, your arm quit going all the way up.

Maybe there was a fall. Maybe a sling for 6 weeks. Maybe nothing happened at all; you just started favoring that side one day and never stopped. And now the top shelf is a two-handed reach, putting on a jacket takes a second longer, and you've quietly rearranged your kitchen around it.

Here is something that surprises almost everybody at their first consult.

If your forehead lines are what bother you, treating just the forehead is usually not the right answer.

The muscle across your forehead is the one that lifts your eyebrows. The muscles between your brows, the ones that make the "11s," pull them down. They work against each other all day long. So if we relax the lifter and leave the pullers alone, some people end up with a brow that feels heavy or sits lower than they like. Nobody wants that, and it is completely avoidable.

That is why Rebecca looks at the whole upper third of the face before she plans anything. She will ask you to raise your brows, then frown, then relax, and she is watching how strong each muscle is and how they balance against each other on your particular face. Then the plan gets built around that, not around a standard number of units in a standard spot.

It is also why we do not love the idea of treating one small area in isolation just because that is the area you pointed out. The lines you see are the result of a system. We would rather treat the system.

None of this makes it complicated for you. You come in, you talk, she maps it out, you decide. Your first visit with Rebecca is a complimentary consultation with no pressure to book anything. People ask us what a peptide visit is like, and I think they expect it to be quick. Walk in, pick something off a list, walk out.

It is not that, and it should not be.

A peptide is a short chain of amino acids. Your body already makes hundreds of them and uses them as signals, little messages that tell tissue to do something. Peptide therapy is the idea of using that same kind of signaling molecule on purpose. Which means the first real question is not which peptide. It is what is actually going on with you.

So here is how Sarah runs a first visit.

She takes a full history. Not a form. A conversation. What you are dealing with, how long, what you have already tried, what a good day looks like versus a bad one. What you are taking, including supplements, because those matter more than people expect.

She looks at the foundations first. Sleep, food, movement, alcohol, stress, and whether anything on your medication list could be part of the picture. If something there is off, that is where the work starts. Peptides do not substitute for that, and anybody telling you otherwise is selling something.

Then labs, ordered based on what your history actually points at rather than a blanket panel for everybody.

And then, only then, a conversation about whether peptide therapy makes sense for you at all. Sometimes the honest answer is not right now, or not this. Sarah will tell you that.

It is worth being plain about one more thing. Several of the peptides people read about online are not FDA-approved drugs in the United States, and the rules around which ones can be compounded have been shifting. That is not a reason to panic. Most exercise advice gets aimed at your heart. Walk more, get your steps in, ride the bike. All good, and we are going to keep telling you to do it.

But the thing that quietly decides how your next 20 years go is muscle.

Muscle is what gets you up off the floor after you have been down there playing with a grandbaby. It is what keeps a stumble from turning into a broken hip. It helps your body handle sugar better. And somewhere around our 40s, we start losing a little of it every year unless we give the body a reason to hang on to it.

The guidance has said the same thing for a long time. Two days a week of something that makes your muscles work. Earlier this year the American College of Sports Medicine released its first major resistance training update in 17 years, and the headline was refreshingly plain. The biggest gain of all is not from the perfect program. It is from going from nothing to something.

Two days. 20 minutes. A set of resistance bands in the living room. A couple of dumbbells in the corner of the shop. Push-ups against the kitchen counter. Standing up out of a chair 10 times without using your hands. That last one counts, and yes, it is harder than it sounds.
