Ever feel unwanted ge***al arousal sensations that show up out of nowhere, without desire, and do not go away?

If you have been told you might have Persistent Ge***al Arousal Disorder (PGAD), you are not imagining it. It can be exhausting, confusing, and isolating.

PGAD often involves sensations like throbbing, tingling, pressure, burning, or fullness in the ge***als that can last for hours or longer. For many people, it is irritating or painful rather than pleasurable. Sitting, stress, or certain movements can make it worse.

One common piece of the puzzle is the pelvic floor. These are deep muscles that support the pelvis, bladder, and reproductive organs. When they stay tight or develop trigger points (irritable knots), they can refer sensation into the v***a, cl****is, or perineum, even when nothing sexual is happening.

Nerves can be involved too. If nerves that travel through the pelvis, like the pudendal or genitofemoral nerve, become irritated or compressed, they can send “too much” sensory information. Over time, the nervous system can become more sensitive and start amplifying normal input.

In our clinic, we often take a pelvic floor and neuromuscular approach. Depending on your symptoms, options may include external dry needling to help release tight pelvic and hip muscles, hands-on myofascial work, and acupuncture to support nervous system regulation and reduce hypersensitivity.

Care is individualized and trauma-informed, and we often coordinate with pelvic PTs, gynecologists, or neurologists when needed.

Noticing your p***s feels firm or semi-rigid even when you are not aroused, along with pain, burning, or numbness?
If so, you may have heard the term Hard Flaccid Syndrome.

Hard Flaccid Syndrome (HFS) is not typically caused by an infection or a hormone problem.
More often, it is linked to tension and irritation in the pelvic floor, the muscle group that supports the bladder, bowel, and sexual organs.

When these pelvic floor muscles stay “guarded” and tight, they can press on nearby nerves and affect circulation.
That can contribute to symptoms like discomfort in the p***s or perineum, changes in sensitivity, and symptoms that get worse with sitting, stress, or exercise.

Another factor can be fascia, the connective tissue that surrounds muscles.
If the fascia and muscles in the pelvis, hips, or lower abdomen are restricted, it can add to nerve sensitivity and that “full” or pressured feeling some men describe.

One treatment we may consider in a pelvic floor dysfunction picture is pelvic floor dry needling.
Dry needling uses very thin, sterile needles to release trigger points and help tight muscles relax, including deeper pelvic muscles that can be hard to reach with other approaches.

In clinic, this is often paired with basics that matter, like breathing and posture retraining, and sometimes acupuncture to help calm an over-alert nervous system.
Care is individualized, and when appropriate we also coordinate with pelvic PTs or urologists.
