Advocate Community Providers (ACP) unites 2,000 individual, independent physicians with 950 hospital partners and community-based organizations dedicated to health, wellness and social services in lower-income, underserved Hispanic, Asian and African-American communities throughout New York City. th North Shore-Long Island Jewish Health System, Inc. ACP’s mission is to convene safety-net providers, partners and other stakeholders to create a culturally competent, patient-centered, comprehensive and coordinated service care model to meet the needs of individuals and families participating in Medicaid. ACP is one of 25 Performing Provider Systems (PPS) in New York’s Delivery Reform Incentive Payment (DSRIP) program to transform the State’s Medicaid system. DSRIP’s goals include reducing avoidable hospitalizations and ensuring that patients and their families with Medicaid switch to a community-based care system that emphasizes preventative care over reliance on the emergency room.

Back-to-school season is right around the corner! Between shopping for supplies, packing lunches, and setting early alarms, it always feels like a fresh start.

It’s the perfect time to reset routines, but it doesn’t have to be overnight, just small changes make a great difference:
🍎 Swap out high-sugar snacks for real food to keep energy and focus all day long.
😴 Start pulling bedtimes back a couple weeks ahead to get used to the new schedule

ABC announced that SOMOS Community Care Founder and Chairman Dr. Ramon Tallaj has been named the Grand Marshal of the 44th Annual National Dominican Day Parade. This year’s celebration will take place on Sunday, August 9, at 11:00 a.m. along Sixth Avenue in Manhattan.

Celebrating its 44th anniversary, this year’s theme, “Dominicanos Identidad y Cultura,” honors the legacy of the Dominican Republic and its deep cultural and civic ties to the United States.

Breastfeeding is a powerful way to support your baby’s immune system, promote healthy growth, and build a lasting bond. 🤱

While it is a natural process, we know it isn't always an easy one. Whether you are navigating the first few days or are well into your journey, here are a few reminders:

✅ It’s a learning curve for both you and your baby. It’s okay to take your time and ask for help.
✅ Keep yourself hydrated and eat balanced meals to support your energy and milk supply.
✅ Build a support system, whether it’s family, friends, or healthcare providers, having a team behind you makes a world of difference.

And remember, every journey is valid. How you choose to feed your baby is a personal decision. What matters most is a healthy baby and a healthy mom.💕
