SOMOS Community Care SOMOS Community Care is a network of community providers improving health care for New Yorkers under (NSLIJ).

Advocate Community Providers (ACP) unites 2,000 individual, independent physicians with 950 hospital partners and community-based organizations dedicated to health, wellness and social services in lower-income, underserved Hispanic, Asian and African-American communities throughout New York City. Representing 644,000 patients across four boroughs of New York City, ACP was created in partnership wi

th North Shore-Long Island Jewish Health System, Inc. ACP’s mission is to convene safety-net providers, partners and other stakeholders to create a culturally competent, patient-centered, comprehensive and coordinated service care model to meet the needs of individuals and families participating in Medicaid. ACP is one of 25 Performing Provider Systems (PPS) in New York’s Delivery Reform Incentive Payment (DSRIP) program to transform the State’s Medicaid system. DSRIP’s goals include reducing avoidable hospitalizations and ensuring that patients and their families with Medicaid switch to a community-based care system that emphasizes preventative care over reliance on the emergency room.

We often hear the term “OCD” used to describe being tidy or organized, but the reality of the disorder goes far beyond a clean desk.

Obsessive-Compulsive Disorder is a cycle of intrusive, unwanted thoughts (obsessions) and repetitive behaviors (compulsions) that a person feels driven to perform to ease their anxiety.

OCD is not a choice or a quirk, it can be exhausting and life-altering. Understanding the difference helps us support those around us with more empathy and less judgment. 💙

Last weekend, SOMOS was proud to participate in La Nueva Yo at Quisqueya Plaza, hosted by Celines Toribio. During the event, SOMOS Director of Behavioral Health, Riquelmy Lamour, joined the panel discussion, “On the Block: Mental Health in the Community,” highlighting the importance of reducing stigma and expanding access to care.

The SOMOS team also provided free health screenings and connected attendees with valuable health resources, reinforcing our commitment to supporting the well-being of the communities we serve.
