DAY HAB at EFMNY

We are currently seeking new people to enroll in our site-based day habilitation program. If your member is seeking a smaller day program, with accommodating hours, and have epilepsy, this is the right program for you.

Qualifications:
• Must have OPWDD Eligibility
• Most recent psychological and psycho-social evaluations
• NOD.09 that states "Site-Based Day Hab"
• Most recent Life Plan
• Most recent Level of Care Eligibility (LCED)
• Insurance card
• New York State ID
