This patient is on her fourth phase of a custom acne scar treatment plan. She has rolling scars with significant tethering and thick, raised edges that have been resistant to less aggressive approaches.

Being Fitzpatrick 2, her skin can tolerate higher energy settings, but that advantage only matters if we use it correctly. We have spaced her treatments over several months to allow each layer of collagen to mature before advancing. Rushing the process in lighter skin can lead to over-treatment and hypopigmentation.

In this phase, I am using pinpoint Erbium laser to target the thick edges of her rolling scars. This tool is incredible for this scar type because it allows me to ablate only the raised border tissue without touching the surrounding skin. The precision means I can smooth the transition between scar and normal skin, reducing the shadowing that makes rolling scars look deeper than they are. Each pass is controlled, each setting calibrated to her specific scar density and healing response.
Another important step in this phase included Radiesse for volume restoration which is a diluted calcium-based biostimulator. We decided to use Radiesse to address the laxity and volume loss in the lower face because it will stimulate both collagen and elastin production to help restore firmness and structural support over time.

Rolling scars require release, refinement, and volume. We have addressed the tethering in previous phases. Now we are refining the surface architecture. The next phase will focus on final blending and any remaining volume restoration.

A common question from my lives: how much improvement can you expect after a single session?

The honest answer is that it depends on several factors. What procedures are included in that session, the location of the scars, and the underlying tissue quality all play a role. The lower face, for example, presents unique challenges. We can perform subcision to release tethered bands, but we often cannot place much volume there initially. Too much filler too soon can cause swelling and puffiness that distorts natural contours. That means the lower face sometimes requires slower, phased volume restoration before the full improvement becomes visible.

In those areas, we may use Bellafill or a diluted Radiesse in later sessions. Radiesse is calcium-based and particularly useful for stimulating elastin and collagen production, which helps firm skin that has lost its tautness. Sculptra also works well in the lower face for gradual, natural volume rebuilding. The first session lays the groundwork. The following sessions build the structure.

Single session results vary. Some patients see significant lifting and smoothing. Others need the full sequence before the transformation becomes apparent. We map this out during consultation based on your specific scar pattern, facial anatomy, and how your tissue responds.

This patient drove down from San Francisco for his consultation, and I am grateful for every mile our patients travel to see us. Over sixty percent of our patients come from out of state, and many of them also tune into my weekly live streams. That constant support means everything.

During his consultation, I used handheld lights to examine his scars under direct illumination. This reveals what photos cannot. I pointed out rolling scars with significant fat destruction across the cheek, tethered bands pulling the skin down, and thickened scar edges that create shadowing. When he smiled, the movement exposed exactly where the tethered fibers were anchored and how the adjacent skin compensates by thickening.

That level of detail is why we take a custom approach. Each scar has a unique depth, shape, and relationship to surrounding tissue. The handheld light allows me to explain why certain treatments are necessary. For him, this means subcision to release the tethering, laser to smooth the thick borders, TCA for the deepest pits, and Sculptra to gradually revolumize the fat loss. We spread out volume sessions to avoid overfilling during the initial treatment. The plan is strategically built around how his tissue moves and heals.

If you saw my live stream last night you got an early preview of this patient’s result who came to us struggling with both active acne and the scars it left behind. We started by addressing the source: controlling her oil production with the Accure laser to get her active breakouts under control first. You cannot effectively treat scars over inflamed, active acne, so stabilizing the skin was our priority.

Once her acne was calm, we moved into scar revision. TCA CROSS targeted her deep ice pick scars, rebuilding from the base. A phenol peel followed, providing significant tissue contraction and collagen response that smoothed the broader surface texture. The improvement after these steps was already substantial.

For the remaining scar tissue, we built a custom plan involving additional TCA sessions combined with laser treatments. Each session was timed to her healing response, adjusting settings based on how her skin was remodeling. This phased, multimodal approach is how we achieve lasting change rather than temporary fixes.
