Isolation is a health risk.

People with strong social relationships had a 50% greater chance of survival in a study of over 300,000 people. And this shows up in rheumatology specifically. About a third of people with inflammatory arthritis report feeling lonely, and lonelier patients tend to have more active disease.

GLP-1 drugs lower TNF. That's the same target as some of our biggest arthritis biologics.

Does that mean they treat autoimmune disease? Not yet. But the question has moved from "we noticed something" to "let's go test it."

I changed my diet. It did what exercise and healthy weight couldn't.

My LDL had been high for as long as I can remember, despite doing everything else right. Three months on a plant-predominant diet dropped it from 150 to 115. That's when I stopped thinking of food as a side conversation.

For RA specifically, the Mediterranean diet has the most evidence behind it. It's not a replacement for your medication. It's a tool that works alongside it.

You don't have to overhaul everything overnight. Small swaps, kept up consistently, beat a perfect diet you can't sustain.

Rheumatology covers more ground than most people expect.

I see the conditions people have heard of — rheumatoid arthritis, lupus, gout. I also see the ones that don't get much attention, like antisynthetase syndrome or IgG4-related disease, plus a handful of conditions I manage together with ophthalmology, ENT, and surgery.

Rest was the wrong advice.

For decades, people with arthritis were told to protect inflamed joints and wait out the flare. The evidence says otherwise. Exercise doesn't increase disease activity, joint damage, or pain in RA. Function and fatigue improve instead.

Strong muscles protect your joints. Movement fights inflammation, lowers cardiovascular risk, and supports mood and sleep. And the biggest gain comes from going from nothing to something. Ten minutes counts.

New in the Medications series: low dose naltrexone.

At 50 mg, naltrexone treats opioid and alcohol use disorder. At 1.5 to 5 mg, it seems to do something else entirely. Patients with fibromyalgia, autoimmune disease, and chronic pain have used it off label for years.

The evidence is mixed. But it's low risk, well tolerated, and I've seen plenty of patients feel meaningfully better on it.
