Sorry guys, at this point 10:51 am- we physically can not print off anymore records. I do not know the exact process yet but as of now the Medical record company will be reaching out to the ones who did not pick up records within 60-90 days. If you have an appointment with a new provider and have their fax number I can do that for you.
IF YOU CALLED PRIOR to 10:51 then YOUR RECORDS are READY FOR PICKUP!!!

Today is the last day!!!! If you have called for records you must be here by 4pm to pick them up!!! After that you will be at the mercy of the electronic medical record company!!

Good Afternoon. I have officially sent out my retirement letter to everyone who has a Follow My Health, patient portal account. This page will contain updates to the office closure and what to do about further services once we get closer to closing.
